Registered Nurse (RN) – Long Term Care (LTC)

Location: Kankakee, Illinois
Contract Length: 13 weeks
Shift: Nights
Hours: 12-hour shifts, approximately 36 hours per week

Facility & Department Details

• Facility type: Skilled Nursing Facility (SNF) and Long Term Care (LTC)
• Bed count: Census typically 80-95 residents; 20 rooms are not private
• Department: LTC with some SNF coverage
• Electronic Medical Record (EMR) system: PCC
• Nurse-to-patient ratios:

  • RN LTC: 1:25
  • RN SNF: 1:13
  • LPN LTC: 1:25
  • LPN SNF: 1:13
  • CNA: 1:10-15 (aim to keep around 10)

Job Responsibilities

• Administer IV antibiotics and fluids (no titratable drips)
• Perform patient assessments, medication administration, dressing changes, and wound vac management
• Complete admissions and discharges
• Participate in interdisciplinary team meetings and grand rounds (encouraged)
• May float between units but generally assigned to the same unit during a stretch
• CNAs handle vitals and activities of daily living (ADLs), but not blood sugars

Patient Population & Common Diagnoses

• Geriatric patients with feeding tubes, IV antibiotics, post-surgical care, rehabilitation, and long-term care needs
• Respiratory therapy available during the day; nurses provide treatments evenings and weekends
• Respiratory support includes AVAP/CPAP/BIPAP, mostly at night, sometimes continuous

Team & Support

• Charge nurses: RN charge nurses on all shifts, including nights
• Physician coverage: Hospitalist after hours, Nurse Practitioner during weekdays, some private MDs, and 24/7 on-call provider
• Interdisciplinary team includes PT/OT/SLP, respiratory therapists, social workers, MDS nurses, wound care nurse, and dietician
• Pharmacy: On-site 24/7 with Omnicell and medication carts

Scheduling & Work Environment

• Shifts: 12-hour night shifts (5 PM to 5:30 AM)
• Weekend and holiday work: Every other weekend and holiday required
• No call or standby requirements
• Orientation: One 12-hour shift on the floor; additional support available if needed
• Scrub color: Any professional color accepted
• COVID unit: Patients intermixed; both PAPR and N95 masks used

Required Qualifications

• Illinois RN license required and must be active at time of submission (temporary or permanent)
• BLS certification required
• Valid driver’s license required for submission
• Minimum 1 year of RN experience, preferably with geriatrics
• First-time travelers are welcome (prior travel experience not required)

Additional Notes

• Local placements accepted if the clinician’s permanent address is more than 20 miles from the facility
• Floating between units possible but generally kept on the same unit during a stretch
• No separate COVID unit; PPE includes PAPR and N95 masks
