Position Details

  • Employment Type: Travel/Contract
  • Contract Length: 13 weeks
  • Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 5:00 PM)
  • Patient Ratio: 1:1

Required Qualifications

  • Valid Physical Therapist license in Texas
  • Minimum 1 year of experience as a Physical Therapist
  • Physical Therapist experience within the last year

Compliance and Documentation Requirements

  • Government-issued photo ID (Driver’s License or State ID)
  • Social Security Card
  • E-Verify attestation
  • Background Check within 90 days of start, covering all locations lived/worked in past 7 years, updated annually
  • Education Verification (degree/diploma/transcript as applicable)
  • References: Minimum 2 references covering 1 year of employment in a comparable specialty/setting within the last 3 years
  • Employment Application covering at least last 5 years
  • Physical Exam within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness to work
  • Drug Testing: 9 Panel Drug Screen within 12 months of start (instant acceptable with attestation)
  • Immunizations and Health Documents:
    • TDAP vaccine within 10 years or declination
    • Negative TB test or chest X-ray within 12 months, updated annually
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer or declination
    • Hepatitis B vaccination series or signed declination
    • Varicella vaccination or positive titer or declination
    • Seasonal Flu vaccine during flu season or declination
    • COVID vaccination record preferred; declinations accepted
  • Respiratory Safety Clearance (RespSafety) required for travel candidates, medically cleared annually
  • Fit Mask test required for travel candidates, updated annually (on-site testing available)
  • Photo for badge (passport style with neutral background) required

Uniform/Dress Code

  • Licensed personnel: Solid navy blue uniforms or scrubs (no print tops except in specific pediatric or women’s services units)
  • Unlicensed personnel: Solid hunter green uniforms or scrubs (no print tops except in specified areas)

Additional Notes

  • Floating Policy: Candidate may be required to float to another facility within a reasonable distance; acknowledgment required
  • Modules and Training: Completion of required WorkRamp modules (regulatory, specialty-specific, and facility-specific) prior to start and annually as applicable
  • Epic Access Training: Required for some regions and roles; transcripts must be submitted as proof of completion
  • Competency Exams: Medication and specialty competency exams may be required based on unit and specialty
  • Skills Checklist: Annual update required for applicable units
