Speech and Language Pathologist (SLP) - Long Term Care (LTC)

Job Description

• Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders.
• Develop and adjust plans of care using standardized assessments and evidence-based practices.
• Assess patient progress and modify treatments accordingly, documenting all clinical and regulatory information in electronic records.
• Evaluate treatment outcomes and make referrals based on results.
• Provide consultation and counseling to patients, families, caregivers, and other service providers.
• Communicate effectively with supervisors and healthcare team members regarding patient progress and treatment plans.
• Prepare comprehensive discharge summaries per regulatory and clinical requirements.
• Accurately record treatment minutes in compliance with all regulations.
• Participate in patient care conferences, utilization reviews, rehabilitation meetings, and required trainings or in-services.
• Obtain necessary durable medical equipment (DME) or supplies as needed.
• Supervise and support Clinical Fellowship Year (CFY) or student Speech-Language Pathologists.
• Adhere to ASHA and state practice acts, code of conduct, and ethics.

Required Qualifications

• Valid Speech Language Pathologist license in Oklahoma.
• Minimum 1 year of Speech Language Pathologist experience within the last year.
• Certification of Clinical Competence (CCC) from ASHA.
• Experience in Long Term Care (LTC) and Rehabilitation settings.

Additional Information

• Work schedule is 8 hours per day during the day shift.
