Surgical Technician Position - Westerville, Ohio
Job Details
- Specialty: Surgical Technician
- Location: Westerville, Ohio
- Shift: Day shift, 07:00 AM - 3:30 PM
- Contract Length: 13 weeks
- Employment Type: Long-Term Assignment (LTA)
Required Qualifications
- Surgical Technician with at least 1 year of experience, including experience within the last year
- Current Basic Life Support (BLS) certification issued by the American Heart Association
Required Onboarding Documentation and Compliance
- Current BLS certification card (front and back images).
- License/certification verification dated within 30 days of start.
- Fit test documentation.
- Negative 10-panel drug screen completed within 30 days of start (including methadone, phencyclidine, benzodiazepines, cocaine, propoxyphene, amphetamines, opiates, 6-acetyl morphine, methaqualone, barbiturates; no THC).
- Signed Code of Conduct form.
- Completed I-9 form.
- Confidentiality and Information System Security Agreement signed.
- Integrity and Audit Services form signed shortly before start.
- Specialty-specific competency testing completed within the last year.
- Color vision test documentation.
- COVID-19 vaccination proof (including booster through September 2025), or approved declination/exemption (declination not accepted in CA or IL).
- Proof of MMR vaccination (2 doses) or IgG titer lab report.
- Hepatitis B vaccination series started prior to start date (titers not accepted).
- Influenza vaccine proof or declination/exemption for current flu season (declination not accepted in CA or IL).
- Tuberculosis screening: 2-step PPD or IGRA test; if positive, chest x-ray per regional requirements.
- Varicella vaccination proof (1 dose) or IgG titer lab report.
- Physical/health statement confirming fitness to work within the last year.
- Criminal background check covering federal, nationwide, and county records for the last 7 years, including SSN verification, dated within 30 days of start.
- Education verification of highest academic level (degree or diploma/GED).
- Exclusion screenings including EPLS, OIG, NSO, and GSA reports dated within 30 days of start.
- Joint Commission CORE exam completed within the last year.
- System access form completed and uploaded one week before start.
- Skills checklist provided and completed by agency for clinical contractors.
Additional Notes
- Resume, skills checklist (within one year), and two professional references are required for submission.
- All documentation must be uploaded to the credentialing system prior to onboarding as specified.
- Declinations for vaccines are not accepted in California or Illinois.
- Drug screen must be completed and resulted before start date with Medical Review Officer (MRO) signature.
This position requires full compliance with all credentialing, health, and professional standards prior to start.
