PT Home Health - Portland, OR (Travel/Contract)

Job Details

  • Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
  • Contract Length: 13 weeks
  • Work Setting: Home Health (visiting patients in their homes)
  • Patient Load: May visit up to 6 patients per day; 1:1 ratio; 30 productivity points standard
  • Weekend Requirement: No weekends required
  • On-Call: Not required
  • Floating: Not required
  • Block Schedule: Not accommodated
  • Locals Accepted: Yes, within 50-75 mile radius considered local

Required Qualifications

  • Valid Physical Therapist license in Oregon
  • Minimum 1 year of Physical Therapist experience within the last year
  • Experience in a Home Health setting
  • Experience with OASIS documentation
  • Basic Life Support (BLS) certification required
  • Must have own reliable transportation; daily travel radius up to approximately 60 miles (60-90 minutes total drive time)
  • Primary COVID-19 vaccination series plus one booster required

Work Environment & Facility Information

  • Attire: Business casual; scrubs allowed but not required; no jeans
  • Charting System: EPIC Remote Client (laptop) and Rover app (phone), plus OASIS documentation
  • Parking: Free parking available
  • Mileage Reimbursement: IRS standard mileage reimbursement provided
  • Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)

Compliance & Onboarding Requirements

  • Background Checks: Comprehensive background check within 90 days of start, including SSN trace, education verification, motor vehicle record, and state-specific searches as applicable
  • Drug Testing: 9-panel drug screen within 12 months of start; instant tests acceptable with attestation
  • Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO; must indicate ability to work with or without accommodation
  • Immunizations & Health Screenings:
    • TDAP vaccine within 10 years or declination accepted
    • Negative TB test or chest X-ray within 12 months, updated annually
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer required
    • Hepatitis B vaccination or signed declination required
    • Varicella vaccination or positive titer required
    • Seasonal flu vaccine required during flu season or declination accepted
    • COVID-19 vaccination record preferred; declinations not required but unvaccinated must follow masking policies
  • Respiratory Safety Clearance: Required for travel candidates; annual medical clearance for respirator use
  • Fit Mask Testing: Required for travel candidates; annual renewal; onsite testing available if needed

References & Employment History

  • Minimum of 2 professional references covering at least 1 year of employment in a comparable specialty/setting within the last 3 years
  • Employment application covering at least the last 5 years required; must include month/year format for dates worked
  • Work history verification may be required if reference requirements are not met

  • First-time travel therapists are accepted
  • No weekend or on-call shifts
  • Facility groups patient visits to minimize driving time
