Physical Therapist - Home Health (Travel/Contract)

Location: Sarasota, Florida
Employment Type: Travel/Contract
Shift: Day (5x8 hours), 8:00 AM - 5:00 PM
Hours per Week: 40


Job Description

  • Provide skilled home health physical therapy visits (6-8 visits per day) for patients with conditions including med/surg, cardiac, orthopedic (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, Parkinson’s, trauma, amputations (AKA, BKA), respiratory patients, neuro, failure to thrive, falls, weakness, and ADLs.
  • Utilize assessment skills including cardiopulmonary/circulatory, orthopedic, neurological, and equipment/devices (gait belt, rolling walker, sit-to-stand lift, sliding boards, collars, supplemental oxygen, prosthetics, wheelchair assessment).
  • Develop and implement home exercise programs, provide patient and family education, and document signs and symptoms of physical, sexual, and psychological abuse.
  • Ensure safety and injury prevention during care.
  • Must have home health experience; OASIS training preferred.
  • Weekend work required 1-2 weekends per contract.

Contract Length

  • Approximately 20 weeks

Required Qualifications

  • Valid Physical Therapist license in Florida
  • Minimum of 1 year of experience as a Physical Therapist
  • Physical Therapist experience within the last year
  • Basic Life Support (BLS) certification
  • Experience in a Home Health setting

Required Documentation & Compliance

  • Current physical exam (within 6 months).
  • Negative TB test (PPD, Qgold, or T-spot) within 10 weeks of start; if prior positive, chest X-ray within past year plus completed TB questionnaire. TB questionnaire must be completed annually.
  • Immunizations or titers for Measles (Rubeola), Mumps, Rubella, Varicella, Hepatitis B (titer, series, or declination).
  • Standard 9-panel drug test within 30 days (instant tests not accepted).
  • Background checks including 7-year history, EPLS/GSA/SAM, HHS/OIG, and education verification.
  • Florida AHCA Level II fingerprint clearance required prior to start (Livescan electronic fingerprinting).
  • Two professional references from within the past 2 years.
  • Employment application and skills checklist must be submitted.
  • Photo for badge with solid background required prior to start.
  • Personal or auto insurance must be current and active during assignment.
  • Completion of BayCare online learning modules prior to start (approx. 4-8 hours).
  • Compliance with BayCare dress code, tobacco policy, and mask fit testing acknowledgment.
  • Respirator medical clearance if respirator use is required.

Competency & Certification Requirements

  • Medication/Pharmacology competency exam (score ≥80%).
  • Home Health RN medication exam for RNs or Long Term Care pharmacology exam for LPN/LVN as applicable.
  • Color blind testing (Relias) prior to start.
  • Dysrhythmia/EKG competency exam required for RNs (not required for PT).
  • Stroke education CEUs and NIH Stroke Certification as applicable (check unit requirements).
  • Crisis Prevention Institute (CPI) certification if applicable (not typically required for PT).

Additional Information

  • Scrubs: Any solid color scrub allowed.
  • Laptop, MiFi, and portable printers provided for documentation.
  • Documentation systems include BayCare Forms, BayCare Scheduling, and Ace Timesheet.
  • Minimum 6 productivity points per day, with possible increase at manager’s discretion.
