Description
Occupational Therapist (OT) - Outpatient
Location: Cheraw, South Carolina
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Description
- Conduct patient evaluations using history, observation, subjective and objective measurements.
- Develop individualized plans of care tailored to patient learning styles and therapeutic needs, considering psycho-social, cultural, and spiritual factors.
- Provide therapeutic interventions for a variety of physical and/or functional limitations including musculoskeletal, neurological, cardiovascular, and developmental impairments.
- Document patient progress with measurable and functional goals.
- Maintain patient records according to policy, including evaluations, re-evaluations, progress notes, treatment notes, charges, and discharge summaries within departmental timelines.
- Adjust schedules to accommodate changes in caseload.
- Supervise specific treatment teams and collaborate regularly with Occupational Therapy Assistants (OTA) to discuss patient care and goals.
- Contribute to departmental performance through professional development activities such as competency training, clinical instruction, in-service coordination, mentoring, program leadership, meeting attendance, and performance improvement initiatives.
- Communicate effectively with physicians, medical personnel, and interdisciplinary team members regarding treatment, patient progress, complications, and recommendations.
- Use appropriate health and safety equipment to protect patients and staff.
- Maintain a clean, orderly, and safe work environment.
Required Qualifications
- Valid Occupational Therapist license in South Carolina.
- Minimum of 1 year of Occupational Therapist experience, including experience within the last year.
- Basic Life Support (BLS) certification.
- Certification in CRCST, CBSPD, or NBSTSA.
Health and Compliance Requirements
- Proof of immunizations or titers with lab printouts (no declinations accepted) for:
- MMR (2 immunizations or titer)
- Hepatitis B (3 vaccinations or titer)
- Varicella (2 immunizations or titer)
- Influenza vaccine required September through April (medical/religious exemptions considered with physician or traveler statement).
- Tuberculosis screening:
- 2 Step PPD or IGRA (Quantiferon/Tspot) with timing requirements (1st within 1 year, 2nd within 30 days of start) or negative Q-Gold/Tspot within 90 days prior to start.
- Annual negative PPD, TB questionnaire, or chest X-ray if applicable.
- 10-panel drug screen completed within 30 days of start (rapid drug screen not accepted).
Professional Documentation Required
- Valid driver’s license or state ID.
- Social Security card.
- Criminal background check within 60 days of start covering last 7 years, including county, statewide, national searches, OIG, EPLS, OFAC, NSOP, SS trace, and education verification; updated annually.
- Employment verification for the past 5 years.
- Resume or professional profile (references preferred but not required).
- Clinical skills checklist completed prior to start and updated annually.
- Confirmation of assignment form (standard form required).
