Physical Therapist (PT) - Temp (Days)

Location: Fort Dodge, Iowa
Contract Length: 13 weeks

Job Summary

• Provide professional and clinical physical therapy care as part of an interdisciplinary team.
• May be required to float within the same discipline across local campuses.
• Work schedule: Days, 8 hours per day.
• Orientation occurs weekly on Mondays.
• Required to work 1-2 of 3 summer holidays (Memorial Day, Independence Day, Labor Day).

Required Qualifications

• Minimum 1.5 years of Physical Therapist experience.
• Experience working with all age groups.
• Home health experience highly preferred.
• Proficient with EPIC electronic medical records system.
• Current BLS certification.
• Valid Driver’s License.
• Must not have been employed by this employer within the last 12 months.
• Skills checklist completed within 1 year of submission.
• License verification and copies of required certifications.
• One supervisory reference required at time of submission.
• Work history/resume required.
• Must have a taxable address outside a 50-mile radius from the facility.

Additional Information

• Normal business hours: 16 hours per week.
• Overtime billable after 40 hours per week.
• Call back pay and on-call pay available with specified minimums.
• Charge/precept pay available.
• Local candidates within 50 miles allowed with a 10% rate reduction (note: pay details excluded here).

Work Environment

• Home health setting.
• May float across local campuses within the same discipline.
