Job Opportunity: Physical Therapist - Home Health

Location: Augusta, Georgia
Employment Type: Travel/Contract
Contract Length: 12 weeks


Job Summary

  • Provide physical therapy services in a home health setting.
  • Patient population includes adults to geriatrics.
  • Typical caseload: approximately 6 visits per day, with an average daily census of 175 patients.
  • Work schedule: Day shift, 5 days per week, 8 hours per day (8:00 AM – 4:30 PM).
  • No weekend shifts required.
  • Floating within facility/scope of practice as needed.
  • Must be available to work major holidays if they fall within the assignment period (expectation varies by number of holidays).

Required Qualifications

  • Valid Physical Therapist license in Georgia.
  • Minimum 3 years of experience as a Physical Therapist, including experience within the last year.
  • Basic Life Support (BLS) certification (AHA or ARC accepted).
  • Experience with home health patient populations including joint replacements, COPD, CHF, and Type 2 Diabetes Mellitus.

Compliance & Onboarding

  • Full compliance with all credentialing and documentation required to start on time.
  • Background check must be completed within 30 days prior to start.
  • Education verification required; diplomas or transcripts may be requested.
  • Physical exam within 12 months of start date required.
  • Negative TB test or chest x-ray within 12 months required; TB questionnaire required annually.
  • Drug screening (10-panel) within 60 days of start required.
  • Immunizations/vaccinations required or documentation thereof:
    • Flu vaccine or signed declination (seasonal).
    • MMR (Measles, Mumps, Rubella) - 2 vaccines or positive titer.
    • Varicella - 2 vaccines or positive titer.
    • Hepatitis B surface antibody titer or signed declination.
    • TDAP highly recommended but not mandatory.
  • Color blindness test required.
  • Skills checklist updated within 12 months required.
  • Two clinical references required.
  • Verification of most recent work history required (can be pending at start with proof it is in process).

Additional Information

  • Scrub color: Any color allowed.
  • Return to office (RTO) requirement: Maximum 7 days.
  • Orientation hours are non-billable and vary based on contract length.
  • Candidate must review and sign facility-specific policies including Code of Conduct and Dress/Professional Appearance Policy.
  • Passport-style photo required for badge.
  • Valid automobile insurance proof required if driving is necessary.
  • No call requirements specified.
