Description
Occupational Therapist - Acute Care Hospital (Travel/Contract)
Location: Long Prairie, Minnesota
Contract Length: 13 weeks
Job Overview
- Work split approximately 60% hospital (general rehab) and 40% outpatient (hands/upper extremity).
- Patient population: Adults of all ages.
- Shift: Day shift, Monday through Friday, 8:00 AM to 4:30 PM (5x8 hours, 40 hours/week).
- Traveler’s primary residence must be at least 25 miles from the facility.
Required Qualifications
- Valid Occupational Therapist license in Minnesota.
- Basic Life Support (BLS) certification.
- Experience with Epic electronic health record system.
Background and Compliance Requirements
- Criminal background check within one year of start date, including:
- 7-year search, SS trace, counties, sex offender, nationwide, and education verification (not required for CNAs).
- International background checks required if candidate has lived/worked outside the U.S.
- Minnesota DHS NetStudy Fingerprint clearance required before start; candidates cannot begin with fingerprints pending unless a “more time needed” letter is approved.
- Must complete the MN DHS Net Study Questionnaire immediately after signing the contract to initiate fingerprinting process.
- Physical exam within 1 year of start date.
- Immunizations and health screenings required:
- Tdap vaccination (no religious declinations accepted).
- Negative TB test or chest X-ray per MN state guidelines.
- Proof of immunity or vaccination for Measles, Mumps, Rubella, Varicella (MMR series or titers).
- Hepatitis B vaccination, positive titer, or declination.
- Seasonal flu vaccination or documented declination (mask required if declined).
- Drug testing: 9-panel test within 1 year of start (instant tests acceptable).
Additional Requirements
- Two professional references from recent employers within the last 2 years.
- Skills checklist required prior to start.
- Completion of facility-specific education modules prior to or during orientation (approximately 16 hours).
- Candidate must sign confidentiality agreement before starting.
- Photo for badge with specific requirements (grey background, no selfies, JPG format).
- Adherence to dress code.
Notes
- Facility does not require annual background check updates unless there is a gap of 30+ days between contracts or background check expires within 30 days of new contract start.
- Fingerprint registration expires after 2 weeks; candidates must complete fingerprinting within this timeframe or be re-registered.
- Candidates may need to travel to a location offering livescan fingerprinting if not available locally.
- Candidates must return badges to management on last day of assignment.
