PT Home Health - Oregon (Portland Area)

Job Details

  • Location: Portland, Oregon and surrounding areas (up to ~60 miles radius, 60-90 minutes total drive time)
  • Employment Type: Travel/Contract
  • Contract Length: 13 weeks
  • Shift: Day shift, 8 hours per day (5x8 hour shifts, 8:00 AM - 4:30 PM)
  • Typical Caseload: 1:1 ratio, may visit up to 6 patients per day; 30 productivity points standard
  • Start Date: ASAP
  • Weekend Requirement: No
  • On-Call: No
  • Floating: No floating between units required
  • Block Schedule: Not accommodated
  • Locals Accepted: Yes, within 50-75 mile radius; locals accepted at same rate
  • Pending License Accepted: Yes
  • Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)
  • Transportation: Traveler must have own transportation

Required Qualifications

  • Valid Physical Therapist license in Oregon
  • Minimum 1 year of Physical Therapist experience within the last year
  • Experience in a Home Health setting
  • Experience with OASIS
  • Basic Life Support (BLS) certification

Health & Safety Requirements

  • Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO; must indicate ability to work with or without accommodation
  • Immunizations/Vaccines:
    • TDAP (Tetanus/Diphtheria/Pertussis) within 10 years or declination accepted
    • Flu vaccine required during flu season (9/1–3/30) or declination accepted
    • COVID vaccination record preferred; declinations not required but unvaccinated must follow infection prevention protocols
    • MMR (Measles, Mumps, Rubella): 2 vaccines or positive titer; declination accepted
    • Varicella: 2 vaccines or positive titer; declination accepted
    • Hepatitis B: Documentation of vaccination series or positive titer or signed declination
  • TB Test: Negative TB test (IGRA/TSPOT, QGold, or 2 PPDs) within 12 months; must be updated annually; positive TB requires chest x-ray documentation
  • Drug Screen: 9-panel drug screen within 12 months; negative results required; marijuana panel excluded
  • Respirator Clearance: Required for travel candidates; annual medical clearance for respirator use
  • Fit Mask Test: Required for travel candidates; annual update; onsite testing available if needed

Background & Compliance

  • Background Check: Within 90 days of start, covering all locations lived/worked in past 7 years; must include SSN trace, death master index, nationwide and county checks, MVR (required for home health), and education verification
  • State-Specific Checks: Washington WATCH search required for border state placements; Oregon Abuse/Neglect search if applicable
  • Education Verification: Required for all candidates; diploma/transcript may be requested if verification is unperformable
  • NURSYS Verification: Required for all RNs (not applicable here but noted for reference)
  • References & Work History Verification: Required as noted above
