Physical Therapist (Home Health PT) - California

Location

• Woodland, CA (service area includes Yolo County and surrounding communities such as Davis, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, Esparto)

Contract Length

• 13 weeks

Schedule

• Day shift, typically 8:30 AM to 5:00 PM
• Approximately 40 hours per week
• Usually rotate Saturdays once a month

Job Description

• Provide physical therapy services according to a written physician's plan of care
• Assess, evaluate, and update therapeutic/rehabilitative/functional status
• Evaluate home environment and assess muscle strength, mobility, gait, range of motion, and rehab potential
• Instruct patients, families, and caregivers on therapeutic appliance use and physical therapy goals
• Report patient reactions and changes in condition to the physician
• Initiate physical therapy programs and instruct other personnel or family members involved in patient care
• May train patients in prosthetic device use
• Supervise homemaker/Home Health Aides involved in rehabilitation
• Identify needs for other home health services and make referrals as necessary
• Prepare and submit clinical and progress summaries
• Provide physical therapy consultation to families/caregivers when indicated
• Participate in peer consultation and ongoing staff competency assessments
• Comply with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and licensure regulations
• Perform other duties as assigned

Required Qualifications

• Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
• Active California Physical Therapy License
• Current BLS (Basic Life Support) certification
• Active NPI number
• Good oral and written communication skills
• Good organizational skills

Additional Information

• Candidates must provide 2 supervisory references: one within the past 12 months and one within the past 3 years
• Candidates may not have been employed (full-time, part-time, or PRN) by certain affiliated health systems within the past year; PRN employment through outside firms is acceptable
• Dress code: scrubs optional; if not wearing scrubs, business casual (slacks/khakis and polo or button-up shirt) is required
• Mileage reimbursement available per GSA guidelines; mileage begins after departing first patient visit (not from housing)
• Mileage timesheets require supervisor approval, with meetings twice a month for signatures or email approval options
