Occupational Therapist - Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (13 weeks)
Shift: 8:00 AM – 4:30 PM

Job Summary

• Provide occupational therapy services to patients in their homes with cognitive and/or physical impairments due to injury, disease, disability, or other causes.
• Patient age range: birth to over 100 years.
• Collaborate with patients, families, physicians, and allied health professionals to assess needs and coordinate care.
• Manage technical, environmental, and financial resources related to patient care.
• Travel required within the geographic service area.

Required Qualifications

• Valid Occupational Therapist license in Massachusetts.
• Minimum 2 years of experience as an Occupational Therapist.
• Occupational Therapy experience within the last year.
• Experience in Home Care setting.
• Experience in Rehabilitation setting.

Required Documentation & Compliance

• Professional license verification (primary source) within 30 days of start; Massachusetts conditional licenses not accepted.
• Submission of a recent color photo (plain background, no filters or eyeglasses).
• Criminal background check, Massachusetts CORI, SSN trace, national sex offender check, and exclusion checks completed within required timeframes.
• Fit test for N95 respirator within 365 days of start; annual updates required.
• Respirator medical clearance attestation if fit test is required.
• Documentation of TB screening within 3 months of start (skin test or blood test) and updated if there is a break longer than 6 months.
• Physician health statement within 365 days of start and updated if break exceeds 6 months.
• Documentation of MMR immunity or vaccination.
• Submission of resume covering past 7 years of work experience including city and state.
• Two professional references within 4 years, including at least one supervisor or higher.
• Completion of skills checklist within 1 year of submission.
• Confirmation of assignment form required upon acceptance and for any start date changes.
• Education verification through background check.
• Health statement and immunization records as required.
• Optional but strongly preferred: Hepatitis B vaccination or immunity proof; Tdap vaccination within 10 years.

Additional Notes

• No religious or medical exemptions allowed for N95 fit testing.
• Travel within the service area is required.
• Must be able to work independently in patient homes.
• Prior travel experience is not explicitly required.
