Physical Therapist Job Opportunity – Indiana

Location

• Kendallville, Indiana
• Serves primarily rural areas across four counties: Noble, Lagrange, Steuben, and DeKalb

Contract Length

• 13 weeks

Schedule

• Monday to Friday, 8:00 AM to 5:00 PM (Day shift)
• 40 hours per week, 8 hours per day

Work Environment

• Mid-sized towns and rural communities
• Approximately 4-6 patient visits daily (point system used)
• Tablet provided for tracking time and visits
• Dress code: Business casual or hunter green scrubs with white or black top

Patient Population and Cases

• Common cases include elderly patients with falls and fractures
• Responsible for Oasis Assessments and Start of Care cases

Required Qualifications

• Valid Physical Therapist license in Indiana
• Minimum of 1 year of Physical Therapist experience, including experience within the last year
• BLS certification issued by the American Heart Association

This position is ideal for physical therapists interested in home health care within a rural setting, managing a moderate caseload with a structured weekday schedule.
