Job Opportunity: Physical Therapist - Home Health
Location
- Lakeport, California

Contract Length
- 13 weeks
Work Schedule
- Day shift, 5 days per week, 8 hours per day (8:00 AM – 5:00 PM)
- Expected patient load: 4-5 patients per day (approximately 5 weighted visits daily)
Required Qualifications
- Valid Physical Therapist license in California
- Minimum 1 year of Physical Therapist experience
- Physical Therapist experience within the last year
- Experience in a Home Health setting
Background and Compliance Requirements
- 7-year background check required within 6 months of start, including:
- Social Security trace with all aliases
- County searches for all locations lived/worked/schooled in last 7 years
- Nationwide sex offender search within 90 days of start
- Education verification of highest level achieved (not required if highest education is high school)
- Motor Vehicle Report (required for driving positions)
- FACIS Level 3 screening within 2 weeks of start and monthly thereafter
- EPLS/GSA/SAM and HHS/OIG checks, updated annually
- Medi-Cal Suspended and Ineligible Provider List search if FACIS III cannot be run
- Employment application must include full 7 years of work history with no gaps
- Employment verification for up to 7 years required; gaps over 30 days must be explained and approved
- Two professional references required (within 3 years of hire)
Certifications and Competencies
- Basic Life Support (BLS) certification preferred (AHA or ARC)
- Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), and other certifications as applicable may be required depending on position specifics
- Medication and competency exams as applicable to profession
Health and Immunization Requirements
- Physical exam within 1 year of start, cleared for work
- Fit mask test within 1 year of start, renewed annually (exceptions apply)
- Adult Tetanus/Diphtheria/Tdap vaccination within 10 years or lifetime TDAP with booster within 10 years
- Negative TB test (QGold, TSPOT, or PPD) within 90 days of start; annual renewal required
- Immunity or vaccination for Measles (Rubeola), Mumps, Rubella, Hepatitis B, and Varicella as per facility policy
- Seasonal flu vaccination required during flu season (Nov 1 – Apr 30) or signed declination; not required for 100% remote employees
- Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids per California law)

- Vendor Training (2024 and 2025 versions depending on contract date)
- General Regulatory Orientation for Contracted Workers

- TB Screening Questionnaire signed by candidate and healthcare provider within 90 days of start; renewed annually
- Personal or auto insurance documentation required for driving positions

- Religious exemptions for certain vaccine or facial hair policies require facility approval and completion of specific exemption forms
- Candidates with positive TB history must provide a negative chest x-ray within 1 year of start
