Description
Physical Therapist - Home Health (Travel/Contract)
Location: Augusta, Georgia
Contract Length: 20 weeks
Job Summary
- Provide physical therapy services in a home health setting, focusing on adult to geriatric patient populations.
- Responsibilities include patient assessments, documentation, and treatment primarily in orthopedics and neurology.
- Average daily caseload: approximately 6 visits, with a census around 80 patients.
- Floating within the facility/scope of practice as needed.
- No weekend shifts required.
- Expected to work major holidays if they fall within the assignment period (see Holidays section).
- Orientation required; number of non-billable orientation hours depends on contract length.
Schedule
- Shift: Day shift, Monday through Friday (5 days/week)
- Hours: 8:00 AM to 4:30 PM (8 hours/day)
- Return to Office (RTO): Maximum 7 days per contract
- No weekend work
Required Qualifications
- Valid Physical Therapist license in Georgia
- Minimum 1 year of Physical Therapist experience within the last year
- Basic Life Support (BLS) certification
Compliance & Screening
- Full compliance with credentialing and onboarding required to start on time; delays in compliance may result in a 2-week start delay due to orientation scheduling.
- Clinical screening will be conducted to determine clinical fit before final approval.
- Background check must be completed within 30 days prior to start.
- Education verification required; official transcripts or diplomas may be requested if verification is pending.
- Drug screening: Negative 10-panel drug screen within 60 days of start, including amphetamines, benzodiazepines, barbiturates, cocaine, opiates, methadone, PCP, propoxyphene, and THC. Instant drug screen acceptable.
- Physical exam within 12 months of start required.
- Annual TB questionnaire required; negative TB test or chest x-ray within 12 months required (with specific protocols if prior positive TB test).
- Immunizations/vaccinations required or documentation of immunity:
- MMR (Measles, Mumps, Rubella) – 2 vaccines or positive titer with booster protocols if needed
- Varicella – 2 vaccines or positive titer with booster protocols if needed
- Hepatitis B surface antibody titer or signed declination
- Flu vaccine or signed declination (required during flu season)
- Tdap (Tetanus/Diphtheria/Pertussis) highly recommended but not mandatory for clearance
- Color blindness testing required.
Additional Requirements
- Valid photo ID required for badge issuance.
- Proof of current automobile liability insurance required.
- Candidate must review and sign facility-specific Code of Conduct and Dress/Professional Appearance policies prior to start.
- Two clinical references required.
- Skills checklist updated within the last 12 months required.
- Verification of most recent work history required; can be pending at start with proof it is in process.
Holidays
If one major holiday falls during the assignment, the traveler is expected to work it. If two major holidays fall during the assignment, both must be worked. If three major holidays fall during the assignment, the traveler must work two of them.

- Scrub color is flexible (any color allowed).
