Physical Therapist - Home Health

Location: San Bernardino, California
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide physical therapy services in the San Bernardino/Ontario home health territory.
  • Day shift: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hour shifts, 40 hours per week).
  • Hours are based on completed visits: 25 visits equal 40 hours; overtime exempt.
  • Guaranteed hours if 25 or more visits are completed.
  • Mileage reimbursement between patients at IRS standard rates.
  • COVID exemptions permitted.
  • Candidates with pending California license will be considered if application is submitted upon submission.

Required Qualifications

  • Valid Physical Therapist license in California.
  • Experience in an Outpatient setting.
  • Experience with OASIS.

Certifications & Competencies

  • BLS (Basic Life Support) - AHA Instructor or Provider acceptable, required for all.
  • ACLS (Advanced Cardiac Life Support) - AHA Instructor or Provider acceptable, required as applicable.
  • PALS (Pediatric Advanced Life Support) - AHA Instructor or Provider acceptable, required as applicable.
  • NRP (Neonatal Resuscitation Program) - required as applicable.
  • EFM (Electronic Fetal Monitoring) - required as applicable.
  • Occupational Therapists are not required to have NBCOT; verify allied licensure requirements as applicable.
  • Medication Test/Pharmacology Competency Exam - required as applicable, minimum 80% passing score or meet/exceed Angoff score.
  • Competency Exam - required as applicable, minimum 80% passing score or meet/exceed Angoff score.

Required Documents & Health Compliance

  • Government issued photo ID (Driver’s License or State ID).
  • Valid passport copy before start.
  • I9 Attestation.
  • Background check including 7-year search, SS trace, county searches, nationwide sex offender check.
  • Annual EPLS/GSA/SAM and HHS/OIG checks.
  • Physical exam within 12 months of start.
  • Immunizations and titers:
    • TDAP vaccine within past 10 years (no declinations accepted).
    • Negative TB test (PPD, QGold, or TSpot) within 1 year; if past positive TB, annual TBQ and chest X-ray within 5 years.
    • Measles, Mumps, Rubella (MMR) proof of immunity or vaccination for candidates born in 1957 or later (no declinations accepted).
    • Hepatitis B titer or vaccine series; declination accepted if within 1 year of start.
    • Varicella immunity proof or vaccination; no declinations accepted.
  • Negative 9-panel drug screen within 1 year (instant drug screen acceptable).
  • Seasonal flu vaccination/documentation required year-round; declinations accepted.
  • COVID vaccination proof or medical/religious exemption required prior to start.

Additional Requirements

  • Passport-style photo (shoulders up, front facing, plain background) for badge.
  • Personal or auto insurance with current expiration dates.
  • Auto registration with current expiration dates.
  • Review and sign training content and hand hygiene policy acknowledgments prior to start.
  • Two professional references required.
  • Employment application and annual skills checklist required for clearance.
