Registered Nurse - Hospice

Location: Morristown, New Jersey
Employment Type: Travel Nursing

Job Description

• Perform patient assessments to determine appropriateness for hospice care and develop individualized care plans for patients and families.
• Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement guidelines.
• Complete admission, discharge, and re-visit assessments and documentation within required timeframes.
• Use age-appropriate teaching methods for patients and families; evaluate and revise teaching plans as needed.
• Manage case responsibilities including developing, implementing, and evaluating evidence-based care plans; prioritize care needs.
• Demonstrate knowledge of hospice regulations, pharmacology, and safe medication management; routinely reconcile medications and maintain current medication orders.
• Participate in interdisciplinary team (IDT) meetings every two weeks and communicate progress toward goals.
• Maintain a safe environment by following infection control measures and reporting infections.
• Participate in performance improvement programs, utilization review meetings, and provide orientation and mentorship to new employees.
• Cross-train to learn new skills and assist with agency growth initiatives as assigned.

Contract Length

• 13 weeks

Shift & Hours

• PM shift (11:00 AM – 7:00 PM)
• 7 hours per day, 37.5 hours per week
• Work every other weekend

Required Qualifications

• Valid RN license in New Jersey
• Hospice experience of at least 2 years, including experience within the last year
• Experience in a Home Health setting
• Associate of Science in Nursing / Associate Degree in Nursing
• BLS certification
