Physical Therapist - Outpatient (Travel/Contract)
Location: Lakeport, California
Contract Length: 13 weeks
Job Description
- Provide physical therapy services primarily in an outpatient setting.
- Plan and implement individualized treatment programs based on physical therapy principles and practices.
- Serve as a resource and provide structured educational opportunities within the facility in your area of specialization.
- Work schedule: Monday to Friday, 8:00 AM to 5:00 PM (5 days x 8 hours), no floating or on-call duties.
Required Qualifications
- Valid Physical Therapist license in California.
- Minimum 2 years of experience as a Physical Therapist, including experience within the last year.
- Current Basic Life Support (BLS) certification.
Background and Compliance Requirements
- 7-year background check required within 6 months of start, including Social Security trace with all aliases, county searches, nationwide sex offender search, education verification, and FACIS Level 3 screening.
- EPLS/GSA/SAM, HHS/OIG, OFAC sanctions, and Medi-Cal Suspended and Ineligible Provider List checks as applicable.
- Education verification of highest level achieved is required (not required if highest education is high school).
- Official transcripts/diplomas may be required if verification attempts are unsuccessful.
- Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to California law).
- Physical exam within 1 year of start, including clearance for work and any work restrictions documented.
- TB screening questionnaire within 90 days of start, renewed annually, with negative TB test or chest x-ray as required.
- Immunizations and/or titers required:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Measles (Rubeola), Mumps, Rubella (MMR)
- Hepatitis B series or signed declination
- Varicella
- Seasonal flu vaccination during flu season (Nov 1 - Apr 30) or signed declination
- Fit mask test within 1 year of start, renewed annually (exceptions apply).
- Respiratory safety clearance prior to start, renewed annually (exceptions apply).
Documentation and Verification
- Must provide full 7 years of work history with no gaps, including city and state of employment.
- Employment verification required for up to 7 years; gaps over 30 days must be explained.
- Two professional references required within 3 years for clinical/allied/per diem acute care candidates.
- Primary source license verification within 30 days of start.
- NURSYS verification required within 30 days for applicable licenses.
- Unit-specific skills checklist required, renewed annually.
Certifications (as applicable)
- BLS (Basic Life Support) - AHA or ARC.
- Other certifications may be required based on position but are not specified for this role.

- Candidate must review and sign facility-specific documents prior to start, including standards of business conduct, code of conduct, vendor training, and regulatory orientation.
