Speech Language Pathologist - Home Health
Location: Owings Mills, Maryland
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Summary
- Perform initial and ongoing patient assessments.
- Provide skilled Speech Language Pathology services.
- Act as case manager for speech therapy-only cases.
- Develop individualized plans of care tailored to each patient’s needs.
- Recommend and teach augmentative alternative communication methods (e.g., automated devices, sign language).
- Teach patients to improve speech, voice, oral/written language skills, and swallowing safety.
- Maintain and update patient records to justify treatment costs for insurance.
- Conduct research related to speech disorders and develop diagnostic/treatment methods or devices.
Schedule & Productivity
- Part-time, ideally 3 days per week (Monday/Wednesday/Friday or Monday/Tuesday/Thursday).
- Shift: Day shift, 8:00 AM to 4:30 PM (3 days, 8 hours each).
- Productivity expectations:
- 6 points per day for 8-hour days.
- 3 points per day for 4-hour days (if applicable).
Required Qualifications
- Valid Speech Language Pathologist license in Maryland
- Basic Life Support (BLS) certification
- Experience in a Home Care setting
Required Documentation & Compliance
- Government-issued photo ID carried during client service hours.
- Proof of E-Verify or attestation.
- Background check: 7-year criminal history including SS trace, counties, nationwide, sex offender, motor vehicle, and FACIS Level 3.
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start and updated with contract renewals.
- Physical exam within the last 12 months (not required annually).
- N95 fit test within 1 year of start, updated annually (can be done onsite).
- Negative TB test or chest X-ray within 1 year, updated annually (with provisions for past positives).
- Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, and Varicella as required.
- 9-panel drug screen within 1 year of start (instant drug screen acceptable).
- Seasonal influenza vaccination or documented declination (collected upon client request or per facility mandate).
- COVID-19 vaccination or approved medical/religious exemption using facility-specific forms.
- Completion of contractor activation form for facility access.
References & Evaluations
- Two professional references within 2 years required for travel/allied positions.
- Performance evaluations required if employed over 1 year; internal evaluations if over 2 years.
- Complete employment application/resume required.
- Skills checklist updated annually as applicable.
Additional Notes
- Must carry photo identification during client visits.
- Must comply with facility-specific COVID-19 policies including vaccination or exemption documentation.
- Auto insurance documentation required.
