Physical Therapist Home Health - Travel/Contract
Location: Dunedin, Florida
Contract Length: 20 weeks
Job Overview
- Provide home health physical therapy services with a patient population including cardiopulmonary, geriatrics, neuro, ortho, and oncology specialties.
- Conduct 6-8 skilled home visits per day (minimum 6 points per day, adjustable by manager).
- Work schedule includes every 3rd weekend.
- Visits and hours are self-scheduled based on office needs.
- Use of laptop, MiFi for internet, and portable printers required for documentation.
- Documentation systems include electronic forms and scheduling software.
Required Qualifications
- Valid Physical Therapist license in Florida.
- Minimum 2 years of physical therapy experience, including experience within the last year.
- Experience in a Home Health setting.
- Basic Life Support (BLS) certification.
- Skills in developing and instructing Home Exercise Programs.
Skills and Competencies
- Assessment and evaluation skills.
- Knowledge of cardiopulmonary/circulatory systems.
- Proficient in documentation and use of equipment/devices such as crutches, gait belts, hospital beds, walkers, slings, sliding boards, collars, and supplemental oxygen delivery.
- Orthopedic and neurological rehabilitation expertise.
- Ability to develop and instruct home exercise programs.
- Medication knowledge and patient/family education skills.
- Awareness of signs and symptoms of physical, sexual, and psychological abuse.
- Safety and injury prevention practices.
Work Hours and Shift
- Day shift, Monday through Friday, 8:00 AM to 5:00 PM (5x8 hours).
- 40 hours per week.
Compliance and Documentation Requirements
- Background checks including nationwide and education verification within 1 year of start.
- Florida AHCA Level II fingerprint clearance required prior to start; must be current and on file.
- Physical exam within 6 months of start date.
- Negative TB test or chest X-ray within 10 weeks of start; TB questionnaire annually.
- Required immunizations or titers: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella. Boosters required for low/equivocal titers.
- Standard 9-panel drug test within 30 days of start (instant tests not accepted).
- Respirator medical clearance annually if respirator use is required.
- Completion of facility-specific online training modules prior to start (approx. 4-8 hours).
- Submission of a clear, professional photo for badge.
- Current personal or auto insurance coverage for home health visits.
- Compliance with dress code and tobacco policy.
- Required facility-specific privacy and affidavit documents.
Additional Requirements
- Two professional references from the past 2 years.
- Employment application and skills checklist must be submitted and available for review.
- Color blindness test required prior to start.
- Medication/pharmacology competency exam as applicable.
Notes
- Locals are accepted at the same rate as travelers.
- Candidates must be prepared to use electronic documentation and scheduling tools.
- Mask wearing is mandatory if flu vaccination is declined.
