Job Description

• Position is for an Acute Care Physical Therapist.
• No floating between units.
• No on-call, weekend, or block schedule requirements.

Required Qualifications

• Valid Physical Therapist license in Texas

Health & Safety Requirements

• Physical exam within 1 year of start, signed by MD, NP, PA, or DO.
• Fit Mask test required for travel candidates (valid within 1 year or completed onsite).
• Immunizations and screenings required:

  • Tetanus/Diphtheria/Tdap (within 10 years; declination accepted).
  • Negative TB test or chest X-ray within 12 months (annual updates required).
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer.
  • Hepatitis B vaccination or signed declination.
  • Varicella vaccination or positive titer.
  • Seasonal Flu vaccine during flu season (declination accepted).
  • COVID vaccination record preferred; declinations not required.
• Drug screening: 9-panel drug screen within 12 months of start.
• Respiratory Safety Clearance required annually for travel candidates.

Background & Documentation

• Government-issued photo ID (Driver’s License or State ID).
• Social Security Card (as applicable).
• Background check within 90 days covering all locations lived/worked in past 7 years; updated annually.
• Education verification required.
• Employment application covering at least last 5 years.
• Minimum of 2 professional references covering 1 year of employment in a comparable specialty/setting within the last 3 years.
• Work history verification required if exceptions to references are requested.
• Photo for badge (passport style with neutral background) required.

Uniform & Parking

• Dress code for licensed personnel: solid navy blue uniforms or scrubs (no print tops except in specific pediatric or women’s services units).
• Free parking available in unrestricted employee/visitor lots.

Additional Notes

• Annual mandatory training modules via LMS (Workramp) including regulatory, specialty-specific, and regional core modules.
• Floating policy acknowledgment required; may be asked to float to other nearby facilities.
• Facility-specific requirements and documentation must be completed prior to start.

This position offers a structured day shift in a Level II Trauma acute care hospital with a focus on physical therapy in an acute care setting. Candidates should be prepared to meet comprehensive licensure, certification, health, and background requirements.
