Physical Therapist - Acute Care

Location: Cottonwood, Arizona
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Details

• Work schedule: 4 days per week, 10-hour shifts (Sunday to Wednesday, 8:00 AM - 6:00 PM)
• Average daily patient census: 11
• Facility type: Short Term Acute Care, Teaching hospital, Level IV trauma center
• Charting system: Cerner
• Dress code: Business casual
• Free parking available
• Guaranteed 40 hours per week; facility may call off up to 6 times during the 13-week assignment
• Floating: May be asked to float to surrounding clinics
• Blocked scheduling in place
• Locals accepted
• Return-to-Office (RTO) requires unit manager approval
• On-call: Not required

Required Qualifications

• Valid Physical Therapist license in Arizona
• Minimum 1 year of Physical Therapist experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association

Required Documentation & Compliance

• Employment application covering last 3 years with documented gaps over 30 days
• Two professional references (one from most recent employer within 1 year)
• Background checks including 7-year criminal search, SSN trace, sex offender search, EPLS/GSA/SAM, and HHS/OIG within 30 days of start
• Physical exam within 1 year confirming ability to perform essential job functions
• Drug test (10-panel) within 30 days of start, updated annually
• Immunizations and titers:

  • Tetanus/Diphtheria/Tdap (within last 10 years or declination)
  • Negative TB test or chest X-ray within 1 year, updated annually
  • Measles, Mumps, Rubella (positive titer or proof of 2 MMR vaccines)
  • Hepatitis B (positive titer, 3-dose series, or declination)
  • Varicella (positive titer or proof of 2 vaccines)
• Seasonal flu vaccination or signed declination form (mask required if declined)
• COVID vaccination series or signed declination form
• Fit mask (N95) test within 1 year, updated annually (PAPR not an option)
• Facility-specific forms including confidentiality agreement, substance abuse screening policy, job description, professional attire and appearance policy, and security question form
• Photo for badge (clear headshot preferred)

Additional Information

• Department operates 7 days a week; weekend work required
• No on-call duties
• Must adhere to facility quality and patient safety standards, including medication administration protocols (BCMA)

This position is suitable for first-time travel therapists.
