Physical Therapist (PT) - Home Health
Location
- Louisville, Kentucky
Contract Length
- 13 weeks
Work Schedule
- Day shifts, typically 5 days per week
- 8 hours per day (40 hours per week)
- Rotating weekend shifts (8 AM - 5 PM)
- Holiday rotation required
Job Details
- Setting: Home Health
- Typical patient diagnoses: Congestive Heart Failure (CHF), Orthopedic conditions, Pneumonia
- Daily workload: Approximately 6.5 visits per day/week
- Responsibilities include start of care (SOC), resumption of care (ROC), recertifications, evaluations, and daily visits
- Orientation: 2 weeks with a preceptor, including a few visits with a work coach
- Attire: Scrubs or business casual
- Equipment provided: Phone and tablet
- Support: Access to clinical manager or educator 24/7 for emergencies and 8 AM - 5 PM for regular support
Required Qualifications
- Valid Physical Therapist license in Kentucky
- BLS (Basic Life Support) Certification
