Schedule

• 3 shifts per week, 8 hours each (Wednesday to Friday, 8:30 AM – 5:00 PM)
• No weekend shifts required
• Facility cannot accommodate 4x10 or 3x12 shifts
• Guaranteed hours with possibility of being called off up to one shift every two weeks

Job Description

• Provide skilled speech-language pathology services in a home health setting
• Typical caseload: 1:1 visits, up to 6 patients per day
• Productivity standard: 30 points per day
• Travel within a maximum radius of approximately 50+ miles; own transportation required
• Charting primarily using EPIC and OASIS (OASIS experience required; EPIC preferred)
• Business casual or scrubs attire

Required Qualifications

• Valid Speech Language Pathologist license in Oregon (pending licenses accepted)
• Minimum 1 year of Speech Language Pathologist experience within the last year
• Experience in a Home Health setting
• Experience with OASIS documentation
• Basic Life Support (BLS) certification
• Must have own reliable transportation
• COVID-19 primary vaccination series plus one booster required
• First-time travel therapists accepted
• Locals within 50-75 miles considered local and accepted at the same rate

Background and Compliance Requirements

• Background check within 90 days covering all locations lived/worked in past 7 years; must be updated annually
• Drug screening: 9-panel drug screen within 12 months of start (instant acceptable)
• Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating ability to work with or without accommodation
• Immunizations and health requirements:

  • COVID-19 vaccination (primary series + booster)
  • Flu vaccine during flu season or signed declination
  • Tetanus/Diphtheria/Tdap vaccine within 10 years or declination
  • Negative TB test or chest X-ray within 12 months, updated annually
  • Measles, Mumps, Rubella (2 MMR vaccines or positive titer or declination)
  • Varicella (2 vaccines or positive titer or declination)
  • Hepatitis B vaccination series or signed declination
• Respiratory safety clearance required annually for travel candidates
• Fit mask testing required annually (travel only)
• References: Minimum 2 references covering 1 year of employment in a comparable specialty/setting within the last 3 years
• Employment application covering at least last 5 years of employment

• No on-call or floating between units required
• Block scheduling not accommodated
• Pending license holders accepted
• RTO (Return to Office) restrictions to be confirmed with hiring manager
• Mileage reimbursement at IRS standard rates
• Facility requires photo ID and badge photo for identification purposes
