Occupational Therapist (OT) - Long Term Care (LTC)

Job Description

  • Screen and evaluate patients with cognitive and/or ADL (Activities of Daily Living) physical functioning disorders.
  • Develop and implement appropriate plans of care following regulatory and clinical practice standards.
  • Utilize standardized assessments and evidence-based practices to support clinical interventions.
  • Provide rehabilitative, skilled, and medically necessary treatment interventions including ADL retraining, therapeutic activities and exercises, modalities, and use of supportive/assistive devices.
  • Assess patient progress and adjust treatments accordingly; evaluate treatment outcomes and modify services as needed.
  • Provide consultation and counseling to patients, families, caregivers, and other service providers.
  • Communicate effectively with supervisors and healthcare team members regarding patient progress, barriers, and treatment plans.
  • Document all required information accurately in electronic records, including treatment minutes.
  • Participate in patient care conferences, utilization review meetings, rehabilitation meetings, and required trainings.
  • Obtain necessary durable medical equipment (DME) or supplies as needed.
  • Supervise and support occupational therapist assistants, rehabilitation aides, and/or students.
  • Adhere to NBCOT and state-required practice acts, code of conduct, and ethics.

Required Qualifications

  • Active Occupational Therapist (OT) license at time of submission and employment.
  • Minimum 1+ years of OT experience.
  • Experience in Long Term Care (LTC) or Rehabilitation settings.
  • New graduates considered if they have completed Level II fieldwork experience (not just Level I observation).
  • Resume, skills checklist (within 1 year), and 0-1 professional references required at submission.

Additional Notes

  • Must comply with all regulatory and clinical practice requirements.
  • Participation in facility and company-required trainings and meetings is mandatory.
