Job Opportunity: MDS Coordinator (Registered Nurse)
Location: Charlotte, North Carolina
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Summary
Seeking a Registered Nurse to serve as an MDS Coordinator in a Skilled Nursing Facility unit within a hospital setting. The unit provides short-term rehab (14-21 days) for residents requiring nursing and medical support, including PT/OT/ST as needed. The role focuses on completing MDS assessments (not a management role), care planning, insurance preauthorization, and utilization review.
Required Qualifications
- Valid RN license in North Carolina
- Bachelor of Science in Nursing
- Minimum 3 years of Skilled Nursing experience, including experience within the last year
- BLS issued by American Heart Association or American Red Cross
- Experience in Long Term Acute Care (LTAC) setting
- Experience in Rehabilitation setting
- Knowledge and experience with Admission Criteria, Appeals and Denials, Benefits Eligibility, Clinical Documentation Improvement
- Ability to determine Medical Necessity per Evidence-Based Guidelines
- Experience working with Physician Advisor roles
- Experience with Pre-Cert Review and Prior Authorizations

Work Details
- Shift: Day shift, 8:30 AM – 5:00 PM (5 days a week, 8-hour shifts)
- Hours: 40 hours per week
- Patient Ratio: 1:12
- Average Daily Census: 9 patients
- Unit Beds: 12
- Call/Floating/Weekend Requirements: None
- Scheduling: Per unit needs
- Orientation: 24 hours on unit after one week hospital orientation
- Scrubs: Navy blue (not provided)
Patient Population & Unit Details
- Patients range from generalized weakness to post-surgical care including surgical wounds and pressure ulcers
- Unit is a Skilled Nursing Facility within the hospital focused on short-term rehab
- MDS Coordinator responsibilities include MDS completion, baseline care plan development, care plan meetings, CAA completion, assessment submission and validation, certification/recertification oversight, diagnosis coding for admissions, education on MDS changes, and utilization review ensuring proper billing documentation
- Additional duties include admission referral review and insurance preauthorization support
Systems & Equipment
- Unit uses Pyxis, EPIC (including EPIC chat, Voalte, and Rover)
- No use of restraints on the unit
Compliance & Background Requirements
- Background checks required within 90 days of start, including local county, federal court, national criminal database, DOJ sex offender search, and education verification for highest degree earned
- Drug screening required within 30 days of start (10-panel, negative result)
- Physical exam within 1 year of start (or within 30 days if past positive TB)
- Fit testing required (N95 or PAPR training)
- Immunizations and titers required for: Rubeola (Measles), Mumps, Rubella, Varicella, Hepatitis B (or facility-specific declination), Flu vaccination or approved exemption
- TB testing: PPD, Qgold, or Tspot within 1 year; if positive, chest x-ray within 5 years and physician-signed TB questionnaire within 30 days of start
- Annual compliance and facility-specific education modules required
- References: 2 professional references within 3 years for travel acute care RNs
Additional Notes
- No call-offs or additional RTO allowed after booking; violations may lead to cancellation
- Interview with manager required prior to offer
- Candidate must complete facility-specific orientation and education checklists
- Candidate must complete Corp ID setup and I-Learn login within 24 hours of receiving credentials to avoid start delays
This position is ideal for an experienced RN with strong MDS assessment skills looking for a travel contract in a skilled nursing rehab setting with no call or weekend requirements.
