Physical Therapist - Outpatient (Travel/Contract)
Location: Berkeley, California
Shift: Day (5 days/week, 8 hours/day, 11:00 AM - 7:30 PM)
Contract Length: 13 weeks
Job Summary
- Provide rehabilitative programs to improve patient mobility, relieve pain, increase strength, and address disabling conditions from disease or injury.
- Perform patient assessment, monitoring, and care with effective communication involving patients, families, and healthcare providers.
- Adhere to all local, state, and federal regulations, policies, and procedures to ensure patient privacy and safety.
- Floating required as needed within the campus.
- Weekend work required on Sundays.
Required Qualifications
- Valid Physical Therapist license in California.
- Basic Life Support (BLS) certification.
Background and Compliance Requirements
- 7-year background check required within 6 months of start, including SSN trace with all aliases, county searches, nationwide sex offender search, education verification, and motor vehicle report if applicable.
- FACIS Level 3 screening required within 2 weeks of start and monthly thereafter.
- EPLS/GSA/SAM and HHS/OIG checks required and updated annually.
- State-specific Medi-Cal Suspended and Ineligible Provider List search required if FACIS III cannot be run.
- Official education verification required unless highest education is high school.
- Transcript/diploma may be required if education verification is unsuccessful.
Certifications and Competencies
- BLS (Basic Life Support) - AHA or ARC only.
- Other certifications may be required based on position specifics (verify with recruiter).
- Medication/Pharmacology competency exam as applicable.
- Competency exams with minimum passing scores as required by profession.
Health and Immunization Requirements
- Physical exam within 1 year of start, including clearance for work and any work restrictions documented.
- Fit mask test within 1 year, renewed annually (exceptions apply).
- Adult Tetanus/Diphtheria/Tdap vaccination within 10 years or lifetime with booster within 10 years.
- Negative TB test (QGold, TSPOT, or 2-step PPD) within 90 days of start; annual renewal required.
- Immunity or vaccination proof for Measles (Rubeola), Mumps, Rubella, Hepatitis B, and Varicella.
- Seasonal flu vaccination or signed declination during flu season (Nov 1 - Apr 30).
- Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to CA law).
- Color vision exam by licensed healthcare provider if applicable.
- Respiratory safety clearance required and renewed annually (exceptions apply).
Documentation and Orientation
- Must complete and sign facility-specific documents prior to start, including:
- Job description acknowledgment
- Standards of business conduct
- Confidentiality agreements
- Report of abuse
- HIPAA agreement
- Orientation booklet and post-test
- Various competency forms specific to Physical Therapy (e.g., Spinal Cord Injury, Intrathecal Baclofen Program, TBI Population, THR/TKR Amputee Multi Frac, Vestibular Competency)
- TB screening questionnaire signed by candidate and healthcare provider within 90 days of start, renewed annually.
- Employment application with full 7 years of work history, no gaps, including city and state of employment.
- Employment verification for up to 7 years required; gaps over 30 days must be explained.
- Two professional references required for clinical and allied acute care candidates.
Additional Notes
- Candidate must be prepared to float within the campus as needed.
- Sunday weekend shifts are required.
- All certifications, background checks, and health documents must be current and compliant prior to start.
- Religious and medical exemptions for vaccinations and fit testing may be considered with proper documentation and approval.
