Description
Physical Therapist (Outpatient) - Travel/Contract
Location: Cades, South Carolina
Specialty: Therapy/Rehabilitation
Contract Length: 13 weeks
Position Details
- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM – 4:30 PM)
- Weekend requirement: No
- On-call requirement: No
- Holiday requirement: Possibly
- May need to float within scope to meet facility needs or between like units/sister facilities
- Facility type: Hospital-based outpatient rehab unit (not on hospital campus)
- Charting system: Epic
- Scrub color: Charcoal gray
- Parking: Free
Required Qualifications
- Valid Physical Therapist license in South Carolina
- Minimum 1 year of Physical Therapist experience, including experience within the last year
- Experience in an outpatient setting
- Basic Life Support (BLS) certification (AHA or ARC only)
Certifications & Competencies
- BLS (Basic Life Support) required, must be valid beyond 30 days from start date
- Other certifications (e.g., ACLS, PALS, NRP) not required for this position unless specified by unit
- Medication/Pharmacology competency exams as applicable
- Must complete facility-specific orientation and safety modules prior to start
Background & Documentation Requirements
- Annual background check covering all locations lived/worked in past 7 years, including:
- SSN trace
- Statewide and national criminal checks
- Sex offender registry
- FACIS 3 and international searches if applicable
- Employment application and resume required
- Proof of 2 clinical references or hospital evaluations
- Performance evaluation dated within 12 months of start
- Work history verification not required for this facility
Health & Immunization Requirements
- Initial physical exam, updated annually, confirming ability to meet physical demands
- Tetanus/Diphtheria/Tdap vaccine within 10 years or declination form with proper documentation
- Negative TB test (2-step PPD or Quantiferon Gold within 90 days of start; chest X-ray if positive)
- Annual TB questionnaire completion required
- Immunity or vaccination documentation for:
- Rubeola (Measles)
- Mumps
- Rubella
- Varicella
- Hepatitis B (titer, vaccine series, or declination)
- 10-panel drug screen within 90 days of start (no instant tests)
- Seasonal flu vaccination or medical/religious exemption required during flu season (Aug 1 – Mar 31)
- COVID-19 vaccination highly preferred; if declined, must complete vaccination declination form
Facility-Specific Requirements
- Must upload a clear, passport-style photo for badge issuance
- Complete and sign facility-required documents prior to start, including:
- Assignment acknowledgment
- Code of conduct review
- Employee confidentiality agreement
- HIPAA confidentiality agreement
- Systems user agreement
- User access request
- Complete all required orientation modules before starting; failure to complete results in being sent home
- During hurricane season (June–November), may be assigned to emergency response teams; avoid confirming new or extension assignments during peak months (August–September) if this is a concern

- Local candidates within 60 miles are not accepted
- Facility may call off 1 shift per 2 weeks
- Respirator medical clearance required annually for patient-facing roles (RespSafety questionnaire)
- Radiation badge not required for this position
