Registered Nurse - Labor & Delivery

Job Summary

• Immediate start
• Must be willing to work as Charge Nurse as needed
• Covers a large geographical area with high acuity patients
• Level III NICU and Newborn Nursery on site
• Approximately 1800 deliveries per year
• Typically involved in 4 scheduled events daily (c-sections, versions, etc.)

Unit Details

• Labor & Delivery: 7 labor rooms, 2 triage rooms, PreOp/PACU in labor room
• OBED: 2 triage rooms
• OR suites: 2
• Postpartum/Antepartum/GYN/Mother-Baby: 25 beds (includes overflow Antepartum monitored by L&D RNs)
• Labor & Delivery and Mother-Baby are physically separate but adjacent units
• Goal designated safe sleep facility
• Rooming-in model for Mother-Baby care

Patient Types / Common Diagnoses

• Pregnancy Induced Hypertension (PIH)
• Gestational Diabetes Mellitus (GDM)
• Pre-Eclampsia, Eclampsia, HELLP syndrome
• Shoulder dystocia
• Postpartum hemorrhage
• Multiples, fetal loss
• Drug addiction
• Cervical ripening cases

Patient Ratios

• Cervical Ripening: 1:1 to 1:3
• Laboring: 1:1 to 1:3
• Active Labor: 1:1
• Couplet/Postpartum/Antepartum: 3 mother/baby couplets or 3 Antepartum/NICU moms

Staffing

• Minimum 6 RNs on Labor & Delivery per shift (day and night), possibly 5 if call out occurs
• 24/7 charge nurse coverage
• Support staff includes unit secretary (days), bed flow facilitator (days), CST 24/7, phlebotomist, anesthesia, respiratory therapy, OB providers, family medicine residents, certified midwife, and RN for delivery care

Required Certifications and Qualifications

• Valid RN license in Maine
• Associate of Science in Nursing / Associate Degree in Nursing
• Minimum 2 years of Labor and Delivery experience, including experience within the last year
• Completion of at least one prior travel assignment in a comparable facility
• BLS (Basic Life Support)
• ACLS (Advanced Cardiovascular Life Support)
• NRP (Neonatal Resuscitation Program)
• Intermediate or Advanced Fetal Heart Monitoring (FHM)
• MOAB (Management of Obstetric Emergencies and Bleeding)
• CST (Certified Surgical Technologist) availability 24/7
• Experience in Charge Nurse roles
• Ability to titrate Pitocin
• Proficiency with fetal scalp electrode placement
• Assist CRNA with epidural placement
• Management of magnesium drips
• Management of shoulder dystocia emergencies
• Circulate in c-section and/or receive baby in OR
• Neonatal resuscitation skills
• Assist with placement and monitoring of intrauterine pressure catheter (IUPC)
• Knowledge and implementation of safe skin-to-skin protocols

Orientation

• 1 day HR/preceptor orientation
• 1 day OR training/PACU
• 1 day preceptor orientation on night shift if contracted to nights

Scheduling

• Shift times: 0700-1930 and 1900-0730
• Weekend rotation: work at least 4 out of 8 weekend shifts (may be back-to-back)
• Holiday rotation required
• No mandatory call, but call pick-up may be available
• Return to Office (RTO) requires manager approval

Floating

• May float to Mother-Baby unit (Antepartum, Postpartum, or occasionally Mother-Baby)
• Newborn Nursery RN handles 24-hour labs

Technology and Equipment

• EMR system: Cerner
• Fetal monitoring: Fetal Link (remote monitoring at bedside and nurse’s station)
• IV pumps: Alaris
• Medication dispensing: Pyxis

Additional Information

• Scrub color: Navy (provided on site)
• 6 credits for scrub machine allotted, scrubs obtained at start of shift; street clothes worn in/out
