Job Opportunity: Home Health Registered Nurse

Location: Salem, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Average patient load: 5-8 patients per day.
  • Types of visits include Start of Care, Wound Care, and Follow-up Visits.
  • Must be willing to travel up to 75 miles from the home clinic, covering Salem and nearby areas (e.g., Canby, Molalla, Woodburn, Colton).
  • Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours).
  • Weekend on-call required approximately 1 weekend every 4 weeks.
  • Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).
  • Dress code: Scrubs.
  • Charting system: Proficiency required in HomeCare HomeBase (HCHB) and OASIS (for RNs).

Required Qualifications

  • Valid Registered Nurse (RN) license in Oregon.
  • Minimum of one year of recent experience in a Home Health setting within the last year.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
  • Experience working with adult patients.
  • Proficiency and experience with HomeCare HomeBase (HCHB).

Productivity Expectations

  • Weekly productivity expectation: 30 units (points system based on visits and activities).
  • Patient assignments typically range from 5-10 patients per day, equating to roughly 6-8 points daily.

Additional Requirements & Compliance

  • Must complete all required background checks including seven-year history, sex offender, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable.
  • Fingerprinting required per state regulations.
  • Health requirements include:
    • Physical exam within 12 months prior to start.
    • Negative TB test or chest X-ray within 12 months.
    • Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
    • 10-panel drug screen.
    • Seasonal flu vaccination or documented declination.
    • COVID-19 vaccination series or documented declination.
  • Must review and sign multiple compliance and confidentiality documents prior to start and annually if assignment extends beyond one year.

Travel & Local Candidates

  • Local candidates accepted at the same rate.
  • Must be willing to travel within a 75-mile radius from the home clinic location.

Notes

  • Weekend and on-call requirements vary; discuss specifics during interview.
  • Some positions may follow a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8 schedules.
  • Candidates must have recent experience in home health nursing and be proficient with required EMR systems.
