Registered Nurse - Home Care

Location: Barre, Vermont
Employment Type: Contract
Contract Length: 16 weeks
Shift: Days (Monday to Friday, some weekend coverage)
Hours per Week: 36

Job Summary

Provide compassionate, personalized end-of-life care in patients' homes, focusing on physical and emotional well-being. Deliver expert pain and symptom management while collaborating with an interdisciplinary team, local physicians, and community providers to optimize patient quality of life.

Required Qualifications

Valid Registered Nurse (RN) license in Vermont
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Minimum of 1 year of recent experience in Home Health or Home Care settings
Experience with OASIS and case management
Basic Life Support (BLS) certification

Key Responsibilities

Provide compassionate hospice care to patients and support to their families
Manage pain and symptoms effectively to ensure patient comfort
Collaborate with healthcare team members and community providers to coordinate care

Additional Information

Schedule includes primarily weekdays with some weekend coverage
36 hours per week commitment
