Speech-Language Pathologist (SLP) - Travel/Contract Position
Location: Stockton, California
Assignment Length: 13 to 26 weeks preferred
Shift: Day shift, 5 days per week, 8 hours per day (40 hours/week)
Start Date: ASAP
Schedule Details:
- May include occasional weekend coverage and floating

Job Description
Provide speech-language pathology services in a skilled nursing facility setting.
Required Qualifications
- Valid Speech Language Pathologist license in California
Required Documentation and Compliance
- Government-issued photo ID (Driver’s License or State ID)
- Emergency Contact Form (required prior to start)
- Background checks including:
- 7-year residential/work/school history
- EPLS/GSA/SAM verification (annual)
- HHS/OIG verification (annual)
- Physical exam within 1 year of start date
- Negative TB test or chest X-ray within 1 year (with specific protocols for past positives)
- Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, or history of chickenpox)
- 9-panel drug screen within 1 year of start (instant drug screen acceptable)
- Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
- Completion of facility-specific compliance quiz (Wage and Rehab Hour Quiz)
- Annual skills checklist and competency exams as applicable (including medication/pharmacology competency)
- National tech certification or state license verification as applicable
References
- 2 professional references required for travel, allied, or per diem acute care roles (within 3 years)
- 1 professional reference required for per diem non-acute care roles

- This is a travel/contract position with a preferred assignment length of 13 to 26 weeks.
- Prior travel experience is not explicitly required.
- Floating and occasional weekend coverage may be required.
