Job Opportunity: RN Rehab - Inpatient Rehab

Location: Waco, Texas
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 6:45 PM – 7:15 AM


Job Description

  • Provide direct patient care in an acute rehabilitation setting.
  • Implement individualized care plans.
  • Coordinate patient discharge needs.
  • Preferred experience in inpatient rehabilitation nursing.

Required Qualifications

  • Valid RN license in Texas
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Minimum of 2 years of rehabilitation nursing experience
  • Rehabilitation nursing experience within the last year
  • Basic Life Support (BLS) certification issued by the American Heart Association
  • NIH Stroke Scale (NIHSS) certification or competency

Onboarding & Compliance Requirements

Professional Documentation

  • Resume or professional profile.
  • At least two professional references.
  • Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and other relevant screenings within 90 days of assignment start.
  • Employment verification covering the past seven years.
  • Signed assignment agreement and confirmation of assignment on agency letterhead.

Testing & Competency

  • Completion of specialty exam with a minimum passing score of 80%, updated every 3 years.
  • Completion of core competencies including HIPAA, Joint Commission, NPSG, OSHA topics, with a minimum passing score of 80%, updated every 3 years.
  • Skills checklist completed within one year of start and updated annually.
  • Epic electronic health record test-out and training checklist with signed attestation.
  • Ascension-specific onboarding education attestation and contract worker onboarding completed prior to or at start.

Health & Immunization

  • Documentation of two MMR vaccines or positive IGG titer; no declinations accepted.
  • Documentation of two Varicella vaccines, serological immunity, or verified history; no declinations accepted.
  • Valid Tdap vaccination within the last 10 years; no declinations accepted.
  • Hepatitis B vaccination series completion or signed declination.
  • Influenza vaccine recommended during flu season (Oct 1 – Mar 31); declinations accepted with mask requirement.
  • Fit test for respiratory protection, preferably completed prior to start.
  • 2-step PPD or IGRA testing for tuberculosis within required timeframes; if positive, chest X-ray and annual TB questionnaire required.
  • Statement of good health or completed physical exam prior to first assignment, updated annually.
  • 12-panel drug screen within 30 days prior to start and annually thereafter.

Identification

  • Valid driver’s license or state ID matching address on file; must show front and back.

Additional Notes

  • Some optional items include unit-specific checklists and badge photo (headshot with white or blue background, no filters).
  • All required documents and verifications must be uploaded or completed as specified prior to or at onboarding.
  • Ongoing compliance with annual updates and re-verifications is mandatory for continued assignment eligibility.
