Description
Hospice Registered Nurse - Vermont, Rutland
Job Type
- Contract
- Duration: 13 weeks
- Hours: 37 hours per week
- Shift: Tuesday - Saturday, 8am-4pm
- Some on-call required
Job Description
- Conduct comprehensive assessments of patient and caregiver needs using the nursing process.
- Develop and communicate realistic, individualized plans of care involving patients and caregivers.
- Participate actively in interdisciplinary team (IDT) meetings to develop and update care plans.
- Serve as Case Manager for assigned patients, managing caseloads and visit schedules.
- Implement medical plans of care, nursing interventions, and procedures to ensure safe, effective, quality care.
- Maintain up-to-date skills in pain control, symptom management, IV use and maintenance, wound VAC, and complex wound care.
- Delegate duties to LPNs within their scope of practice to ensure care plans are followed.
- Evaluate and document patient responses and care outcomes to ensure quality of care.
- Maintain accurate and timely nursing documentation in compliance with evidence-based practice and regulations.
- Perform patient and caregiver education on medication side effects, treatment options, discharge instructions, and reportable conditions.
- Complete required agency, state, and federal forms accurately and timely.
- Conduct supervisory visits and communicate findings promptly.
- Assess needs for Hospice Aide services, coordinate plans of care, and provide ongoing supervision.
- Report patient incidents (e.g., infections, injuries) to Quality Improvement specialists as needed.
- Coordinate safe patient discharges and transfers, ensuring proper documentation.
- Collaborate with physicians, providers, team members, and vendors to support care plans.
- Participate in monthly peer review audits and bi-weekly team meetings.
- Obtain prior authorizations and communicate with insurance case managers regarding services and care plan changes.
- Develop and deliver educational presentations internally and to community agencies.
- Advocate proactively for patient care needs to ensure sensitivity to individual patient and family circumstances.
Required Qualifications
- Valid Registered Nurse (RN) license in Vermont
- Minimum of 2 years of hospice nursing experience, including hospice experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
