Physical Therapist Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 08:00 AM – 04:30 PM
Weekend Rotation: Once every 6 weeks
Job Summary
Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Patient age range varies from birth to over 100 years. Collaborate with patients, families, physicians, and other health professionals to assess needs and coordinate care. Maintain thorough and timely electronic documentation and interdisciplinary communication in accordance with agency policies and standards of practice.
Required Qualifications
- Valid Physical Therapist license in Massachusetts
- Minimum 1 year of Physical Therapist experience, including experience within the last year
- Experience in a Home Care setting
- Experience in a Home Health setting
- Minimum 1 year of case management experience
Onboarding and Compliance Requirements
- Submission of a recent (within 6 months) professional color photo (no selfies, filters, or eyeglasses; plain background).
- Motor Vehicle Record (MVR) check within 45 days of start.
- Primary source verification of professional license within 30 days of start and updated upon expiration.
- N95 respirator fit test within 365 days of start and updated annually (no exemptions allowed).
- Nurse Aide Registry check within 30 days of start (required by MA Department of Public Health for all employees).
- Respirator Medical Clearance attestation (if fit test required).
- Documentation of immunity or vaccination for Measles, Mumps, and Rubella (MMR).
- Tuberculosis (TB) test documentation within 3 months of start and updated if there is a break longer than 6 months.
- Physician health statement signed by MD/DO/NP/PA within 365 days of start and updated if break exceeds 6 months.
- Criminal background check within 45 days prior to start and updated if break exceeds 6 months. Includes county and international residence/employment history for last 7 years.
- Massachusetts CORI background check within 30 days prior to start and updated if break exceeds 6 months.
- Submission of resume including past 7 years of work experience with city and state.
- Two professional references within 4 years, including at least one supervisor or higher.
- Skills checklist completed within 1 year of submission.
- Confirmation of assignment/point of information form required for all initial assignments and extensions.
- Education verification through background check company (highest level achieved).
- Social Security Number trace with criminal check covering last 7 years.
- National sex offender registry check within 30 days of start and updated annually.
- Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days of start and annually.
- Candidate submission coversheet required for initial assignment submissions.
- Beeline attestation completed after registration in the system.
Additional Notes
- Positive TB skin test requires documentation of a negative chest X-ray after the positive test.
- Name change documentation required if credential names do not match legal name or profile.
- No religious or medical exemptions allowed for N95 fit testing.
- Updates to fit testing can be done at specific locations if assignment is extended.
This position requires a dedicated physical therapist with certified home health experience and case management skills, able to provide high-quality care in patients' homes while complying with comprehensive onboarding and regulatory requirements.
