Physical Therapist - Home Health (Travel/Contract)

Location: Roseburg, Oregon
Contract Length: 13 weeks


Job Overview

  • Provide physical therapy services in a home health setting with home visits and periodic office meetings.
  • Average patient load: 5-6 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day (30-32 points per week expected).
  • Typical schedule: Day shift, Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours). Some positions may have a 4x10 schedule (Friday-Monday).
  • Must be willing to travel up to 75 miles from the home clinic, averaging about 100 miles per week. Mileage reimbursed at IRS standard rate (patient-to-patient travel compensated; commute to/from first and last patient is not billable).
  • EMR systems used: PointCare and Homecare Homebase (HCHB).
  • Cases primarily include Start of Care and Follow Ups.
  • Workload demand is due to increased patient census.

Required Qualifications

  • Valid Physical Therapist license in Oregon.
  • Minimum 1 year of recent Physical Therapist experience within the last year.
  • Experience working with adult patients in a home health setting.
  • Experience with Homecare Homebase (HCHB) EMR system.
  • BLS certification required.
