Outpatient Physical Therapist (Travel/Contract)
Location: Charleston, South Carolina
Contract Length: 13 weeks
Job Details
- Shift: Monday to Friday, 7:00 AM – 3:30 PM
- Weekend Requirement: None

Required Qualifications
- Valid Physical Therapist license in South Carolina
- Minimum 2 years of Physical Therapist experience, including experience within the last year
- Basic Life Support (BLS) certification
Compliance & Documentation Requirements
- Copy of Driver’s License and Social Security Card
- Completed and accurate I-9 form with E-Verify attestation
- Annual background check covering all locations lived/worked in past 7 years, including national and state criminal searches
- Education verification or diploma/transcripts as placeholder
- South Carolina Medicaid Excluded Providers search (for sub vendors)
- Proof of 2 clinical references or hospital evaluations
- Complete employment application/resume with all PTO, contract assignments, agency names, and no unaccounted gaps
- Last 3 clinical employers verified through a third-party vendor (attestation letters accepted if verification not possible)
Health & Safety Requirements
- Initial physical exam, updated annually, attesting ability to meet physical demands
- Fit test for N95 mask (O&M Halyard FLUIDSHIELD or 3M Aura 1870+) within 90 days prior to start, renewed annually
- Tetanus/Diphtheria/Tdap vaccine within 10 years or declination form
- Negative TB test (T-spot or Q-Gold) within 90 days or proof of prior positive with chest x-ray
- Annual TB questionnaire
- Required immunizations or positive titers for:
- Measles (Rubeola)
- Mumps
- Rubella
- Varicella
- Hepatitis B (titer, vaccine series, or declination)
- 10-panel drug screen within 90 days prior to start (no instant tests)
- Seasonal flu vaccination or medical/religious exemption (flu season August 1 – March 31; documentation deadline December 5)
- COVID-19 vaccination highly preferred; if declined, must complete vaccination declination form
Facility-Specific Requirements
- Completion of orientation modules (approx. 2 hours) prior to start; must be done on a computer, not a phone
- Upload COVID and flu vaccine documentation into facility platform before start
- Passport-style badge photo required by Wednesday end of day prior to start (frontal view, business casual attire, no hats or sunglasses)
- All requested Return to Office (RTO) restrictions must be submitted and approved before start
- Facility may call off 1 shift per 2 weeks (guaranteed hours with this exception)

- No prior travel experience required
- No floating between units or sister facilities
- Locals accepted on a case-by-case basis with adjusted pay (not included here)
- Radiation badge not required for this position
