Description
RN Endoscopy - Cleveland, Ohio
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 07:00 AM – 05:30 PM
Required Qualifications
- Valid RN license in Ohio
- Minimum 1 year of Endoscopy experience, with experience within the last year
- BLS certification issued by the American Heart Association
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Health and Immunization Requirements
- COVID-19 vaccine series preferred but not required; medical or religious exemptions accepted.
- Positive MMR titers within the last 15 years required; booster protocol applies if low titer.
- Positive Varicella titers within the last 15 years required; booster protocol applies if low titer.
- Tdap vaccination within the last 10 years required; no declinations accepted.
- Hepatitis B: Proof of full series, partial series with signed declination, or positive titer within 15 years accepted.
- 2-Step PPD or IGRA (Quantiferon/Tspot) with negative results within last 6 months required annually. If positive, clear chest x-ray (valid 5 years) and current TB questionnaire required.
- Fit Test for N95 respirators completed within 1 year prior to start, updated annually (cannot be done onsite).
- Physical exam within 1 year prior to start confirming ability to work without restrictions.
- Influenza vaccine required October through March; declination requires mask-wearing and completion of specific forms.
Background and Verification Requirements
- Primary Source Verification of RN license and Nursys verification within 30 days prior to start; must be updated every 2 years with license renewal.
- Criminal background check within 6 months prior to start, including Social Security trace, county searches (7 years), national sex offender registry, and national criminal sweep. International background check required if not living in the U.S. for past 5 years.
- Ohio fingerprinting and FBI background checks required depending on length of Ohio residency; attestation allowed if fingerprints pending.
- Employment verification for the last 5 years through third-party verification; attestations not allowed.
- Education verification through third-party; international education requires equivalency documentation.
- UHS OIG exclusion check and SAM/EPLS/GSA/OFAC screening within 6 months prior to start.
- Drug screen within 60 days prior to start; 10-panel urine drug screen required, instant drug screens not accepted. New screen required if break in assignment exceeds 60 days.
- Completed I-9 attestation confirming eligibility to work in the U.S.
- Non-expired photo ID (Driver’s License or State ID).
Skills and Competency Requirements
- Completion of a skills checklist specific to the clinical area within 1 year prior to start (one-time requirement).
- Competency exam for the relevant department with a minimum passing score of 80%, completed within 1 year prior to start (one-time requirement).
- Completion of core competencies including HIPAA, Joint Commission National Patient Safety Goals, OSHA topics, with a minimum passing score of 80%, completed within 1 year prior to start (one-time requirement).
- Completion of required safety and compliance training modules prior to clearance; renewal annually.
