Registered Nurse - Home Care

Job Description

• Provide case management for home health patients.
• Manage a high volume caseload, seeing 5-6 patients per day.
• Extensive documentation required.
• Work with the South team covering Plainsboro and West Windsor areas.

Contract Length

• 13 weeks

Schedule

• Shift: 8:00 AM – 4:30 PM
• Hours per week: 40

Required Qualifications

• Valid Registered Nurse (RN) license in New Jersey.
• Associate of Science in Nursing or Associate Degree in Nursing.
• Minimum 2 years of home health experience, including experience within the last year.
• Experience in a home health setting.
• Case management experience.
• Experience with OASIS (Outcome and Assessment Information Set).
• BLS certification.

• Driver’s license and car insurance

• Core mandatory annual training
• Color vision screening
• Fit mask test
• Latex health questionnaire
• Resume and references required
