Description
Physical Therapist – Home Health
Location: Rancho Cordova, California
Contract Length: 13 weeks
Job Description
Seeking an experienced Home Health Physical Therapist with a minimum of 2 years of experience. The role involves providing physical therapy treatments in patients' homes, assisted livings, senior apartments, and board and care homes. Patient caseload typically includes 3 to 5 visits per day, including 2 Start of Care visits and 3-4 evaluations or combinations thereof. Diagnoses treated include orthopedic, cardiovascular, pulmonary, oncology, neurological, geriatric, and amputations. No pediatrics or hospice patients.
Work is managed by an RN Manager with support from a Regional Rehab Leader/Educator. The PT will supervise up to 2 PTA/COTAs.
Schedule
- Shift: Day shift, 8:30 AM to 5:00 PM
- Hours per week: 40
- One weekend per month may be requested but is not mandatory
- Holiday work is required, with some flexibility for time off requests
Required Qualifications
- Minimum 2 years of experience in Home Health or healthcare setting (use the higher requirement if discrepancy)
- Active California Physical Therapist license
- Current BLS certification (AHA)
- Active NPI number
- Valid driver’s license and proof of automobile insurance
- Experience with Home Health including OASIS documentation required
- EMR experience with HomeCare Home Base (HCHB) preferred
- Good oral and written communication skills and strong organizational skills
Additional Requirements & Information
- Candidates must not have been directly employed (full-time, part-time, or PRN) by any facility associated with a specific health system within the past year; PRN employment through outside firms is acceptable
- Two supervisory references required at submittal: one within the last 12 months and one within the last 3 years
- Legal name on government-issued ID must match application documents (no middle initials or names)
- Mileage reimbursement available per GSA guidelines; mileage accrues after departing first assignment, not from housing to first visit
- Work environments may be cramped, hot/cold, or cluttered; must be able to adapt to home settings
- Dress code: business professional; closed-toe shoes required; no jeans, tank tops, or shirts with logos; scrubs recommended but not provided
- Orientation: robust 3-week process including online learning and field precepting; solo work expected by end of week 2 or 3 at latest
Patient Care & Territory
- Patients served come from multiple hospitals and community doctors in the service area
- Territories are grouped to reduce excessive mileage and improve efficiency; PTs may cover patients across teams as needed
Summary
This is a home health Physical Therapist position requiring a California license, BLS, and at least 2 years of relevant experience. The role involves autonomous patient care in diverse home environments with a structured orientation and supportive management. The schedule is primarily weekdays with occasional weekend and holiday work.
