Physical Therapist (SNF) - Travel/Contract

Location: Live Oak, California
Assignment Length: 13 to 26 weeks
Shift: Day shift, 5 days per week, 8 hours per day (40 hours/week)
Start Date: ASAP

Job Description

• Provide physical therapy services in a Skilled Nursing Facility (SNF) setting.
• May include occasional weekend coverage and floating as needed.

Required Qualifications

• Valid Physical Therapist license in California

Required Documentation & Compliance

• Government-issued photo ID (Driver’s License or State ID).
• Emergency Contact Form prior to start.
• Background checks including 7-year residential/work/school history, sex offender registry, EPLS/GSA/SAM, and HHS/OIG checks (updated annually).
• State license verification via primary source.
• National technical certification as applicable.
• Medication/Pharmacology competency exam as applicable.
• Facility-specific compliance quiz on wage and rehab hours (California).
• Physical exam within 1 year of start date.
• Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if past positive, negative chest X-ray within 1 year required.
• Immunization or titer proof for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (titer, declination, or vaccine series)
  • Varicella (titer, vaccine series, or history of chickenpox)
• 9-panel drug screen within 1 year of start (instant drug screen acceptable).
• Seasonal flu vaccination or declination during flu season (October 1 - March 31).

References & Competencies

• Two professional references required for travel, allied, or per diem acute care roles (within 3 years).
• One professional reference required for per diem non-acute care roles.
• Annual skills checklist required.

• Prior travel experience is not explicitly required.
