    Description

    Physical Therapist

    Location: San Jose, California
    Employment Type: Clinical-Allied
    Shift: Monday to Friday, 8:00 AM – 4:30 PM

    • Combination of on-site and mobile work depending on participant needs
    • 1-2 days per week dedicated to home visits for home safety and therapy needs
    • Involved in interdisciplinary meetings and care coordination in a PACE setting
    • Must have reliable transportation

    Job Summary

    Provide restorative and rehabilitative Physical Therapy services to frail, elderly participants both at the center and in their homes. Collaborate closely with physicians, rehabilitation staff, and interdisciplinary team members to maximize participant independence and safety. Participate in care planning, supervision of therapy assistants, and support care transitions between settings.


    Key Responsibilities

    • Conduct Physical Therapy assessments to determine care needs and goals
    • Deliver and document interventions including active/passive exercise, muscle re-education, gait and ADL training, and therapeutic treatments (massage, heat, sound, water, light, electricity)
    • Supervise and train Therapy Assistants implementing Functional Maintenance Programs
    • Provide training to staff, participants, and caregivers on therapeutic equipment use and care
    • Manage equipment ordering, tracking, and maintenance
    • Communicate participant condition changes promptly to the interdisciplinary team
    • Support enrollment of prospective participants in coordination with marketing
    • Participate in end-of-life care coordination and support
    • Manage direct reports through training, supervision, and mentoring
    • Complete all required documentation and mandated trainings timely and accurately
    • Maintain confidentiality and follow organizational policies and quality improvement activities
    • Exercise flexibility to meet evolving business needs

    Required Qualifications

    • Valid Physical Therapist license in California
    • Bachelor’s Degree in Physical Therapy
    • Minimum 5 years of Physical Therapist experience, including experience within the last year
    • Basic Life Support (BLS) certification
    • Experience in supervisory roles with demonstrated mentoring ability
    • Experience implementing Functional Maintenance Programs

    Contract Length

    • 13 weeks

    Work Hours

    • 8 hours per day, 40 hours per week

    Additional Information

    • Work setting includes both center-based and home environments
    • Participation in interdisciplinary team meetings and care coordination beyond direct patient care
