Description
Job Opportunity: Home Health Physical Therapist
Location: Manning, South Carolina
Employment Type: Long-Term Assignment (13 weeks)
Shift: 8:00 AM – 5:00 PM
Job Description
- Provide physical therapy services according to physician’s written orders in a home health care setting.
- Daily interaction with patients, family members, agency personnel, and physicians.
- Manage time independently and comply with attendance and reporting requirements.
- Collaborate with staff, students, and external agencies.
- Participate in professional development activities such as competency training, clinical precepting, in-service coordination, mentoring, and clinical education coordination.
- Knowledge of state therapy laws, federal health insurance program regulations, and home health agency conditions of participation required.
- Ability to handle safety issues for staff and patients in all settings.
- Moderate physical activity including walking, standing, bending, lifting (up to 50 pounds), sitting, and driving.
- Must have good manual dexterity, hearing, eyesight, and full range of motion for patient care.
- Strong interpersonal skills, empathy, effective communication with diverse populations, and professional, ethical behavior required.
- Ability to assess patient status across the lifespan and interpret data to meet age-specific needs.
Required Qualifications
- Valid Physical Therapist license for South Carolina (state-specific license verification required).
- Basic Life Support (BLS) certification (American Heart Association only).
- Proof of immunizations with lab documentation (no declinations accepted):
- MMR (2 immunizations or titer)
- Hepatitis B (3 vaccinations or titer)
- Varicella (2 immunizations or titer)
- Influenza vaccine recommended during September–April (medical/religious exemptions considered case-by-case).
- Tuberculosis screening:
- 2 Step PPD or IGRA (Quantiferon/Tspot) within required timeframes, or
- Annual negative PPD or TB questionnaire if applicable.
- If positive PPD, negative chest X-ray within 90 days of start required.
- Valid driver’s license or state ID.
- Social Security card.
- Clinical skills checklist completed prior to start and updated annually.
- Criminal background check including county, statewide, national search, OIG, EPLS, OFAC, NSOP, SS trace, and education verification within 60 days of start covering last 7 years.
- Employment verification for the past 5 years.
- 10-panel drug screen completed within 30 days of start (rapid drug screen not accepted).
- Resume or professional profile submission required; references preferred but not mandatory.
Experience
- Minimum of 5 years of employment verification required (implies relevant experience).
Physical and Professional Requirements
- Ability to perform moderate physical activity and strenuous movements necessary for patient care.
- High degree of maturity, self-motivation, creativity, initiative, and independent judgment.
- Effective interpersonal skills and ability to communicate with diverse populations.
- Familiarity with community resources and referral systems.
Additional Information
- Fit test for respiratory protection equipment recommended within one year of start (optional).
- Must comply with agency policies and procedures.
- Responsible for professional behavior, accuracy in documentation, and follow-up.
