Description
Occupational Therapist - Home Health
Location: Missoula, Montana
Employment Type: Contract
Contract Length: 13 weeks
Schedule: Monday to Friday, 8 AM to 5 PM (occasionally evenings or weekends)
Hours per Week: 40
Job Summary
Provide occupational therapy evaluation and rehabilitation interventions in patients' homes. Manage and supervise patient care, including patient and family education.
Key Responsibilities
- Evaluate patients and establish individualized plans of care for the home environment.
- Coordinate care with other disciplines and obtain orders for additional services as needed.
- Deliver direct patient care based on care plans and physician orders.
- Apply occupational therapy evidence-based practice guidelines, Medicare regulations, and Conditions of Participation during evaluations, re-evaluations (including 30-day functional assessments), and treatments.
- Modify treatment plans as appropriate, adjusting frequency and duration of care.
- Perform CMS-required OASIS scoring and collaborate with Quality team for necessary adjustments.
- Maintain understanding of insurance requirements specific to Home Health.
- Participate in multidisciplinary conferences to develop patient-centered care plans.
- Communicate effectively with physicians and multidisciplinary team members.
- Document all skilled services, adverse events, and relevant communications in the medical record at point of care.
- Engage in Quality Assurance Performance Improvement initiatives.
- Meet productivity standards and maintain accurate time logs and mileage records.
- Follow agency policies for commuting and mileage logging; communicate daily with scheduler.
- Optimize continuity of care and travel routes when scheduling visits.
- Supervise Occupational Therapy Assistants and Certified Nursing Assistants as required.
- Participate in care conferences and plan admissions, OASIS time points, and discharges throughout patient episodes.
- Maintain occupational therapy licensure requirements, including continuing education.
- Adhere to ethical standards as defined by the occupational therapy licensure board and agency policies.
- Participate in agency-provided educational opportunities.
Required Qualifications
- Valid Occupational Therapist license in Montana
Additional Information
- Full-time equivalent (1.0 FTE) position.
- Work primarily during daytime hours with occasional need for evening or weekend availability.
