Job Opportunity: Home Health Registered Nurse

Location: Portland, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Shift: Day shift, 8 hours per day (8:00 AM – 4:30 PM), 5 days a week (40 hours/week)
  • Start Date: ASAP
  • Patient Ratio: 1:1, with up to 6 patient visits per day (30 productivity points standard)
  • Weekend Requirement: No weekends required
  • On-Call: Not required
  • Floating: No floating between units
  • Block Schedule: Not accommodated
  • Locals Accepted: Yes, within 50-75 mile radius
  • Guaranteed Hours: Facility may call off up to one shift every two weeks
  • Transportation: Traveler must have their own transportation; daily travel radius up to ~60 miles (60-90 minutes total drive time)

Required Qualifications

  • Valid Registered Nurse (RN) license in Oregon
  • Minimum of 1 year of Home Health nursing experience within the last year
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Basic Life Support (BLS) certification issued by the American Heart Association
  • Experience with OASIS documentation in a Home Health setting

  • Type of Facility: Home Health
  • Attire: Business casual; scrubs allowed but not required; no jeans
  • Parking: Free
  • Mileage Reimbursement: IRS standard

Compliance & Background Requirements

  • Government-issued photo ID and Social Security Card required
  • Background check within 90 days covering all locations lived/worked in past 7 years, including:
    • SSN Trace, Death Master Index, Nationwide and County checks, National Sex Offender Registry, FACIS Level 3, Education Verification, Motor Vehicle Record (MVR)
  • State-specific searches and verifications as applicable for Oregon and Washington
  • Education verification required for nursing degree
  • Drug screening: 9-panel drug screen within 12 months of start
  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness for duty
  • Immunizations and health screenings required:
    • TDAP within 10 years (declination accepted)
    • Negative TB test or chest X-ray within 12 months (annual updates required)
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer
    • Hepatitis B vaccination or signed declination
    • Varicella vaccination or positive titer
    • Seasonal flu vaccine during flu season (declination accepted)
  • Respirator clearance required annually for travel candidates
  • Fit mask testing required for travel candidates (annual renewal)

Additional Requirements

  • Minimum of 2 professional references covering 1 year of employment within the last 3 years in a comparable specialty/setting
  • Employment application covering at least the last 5 years of employment
  • Must have a valid driver’s license and reliable transportation
  • No weekend or on-call requirements
  • No floating between units

Prior Travel Experience

  • First-time travelers accepted (no prior travel experience required)

Notes

  • Facility cannot accommodate 4x10 or 3x12 shifts
  • Confirm return-to-office (RTO) restrictions with hiring manager
  • Background checks and other compliance documents must be current and uploaded as required
  • Annual regulatory and specialty-specific training modules are generally not required for Home Health at this facility
