Description
Physical Therapist - Home Health (Travel/Contract)
Location: Honolulu, Hawaii (serving South/Central/East parts of the island including multiple communities)
Contract Length: 13 weeks
Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 5:00 PM)
Hours per Week: 40 hours (guaranteed if 25 or more visits are completed)
Job Description
- Field position providing home health physical therapy services across various locations on the island.
- Candidates with home health experience preferred; outpatient experience accepted if no home health experience.
- Must have experience with OASIS documentation.
- Mileage reimbursement follows IRS standard rates.
- COVID vaccination exemptions permitted.
Required Qualifications
- Valid Physical Therapist license in Hawaii
- Experience in an Outpatient setting
- Experience with OASIS
Required Documentation & Compliance
- Government-issued photo ID (Driver’s License or State ID).
- Valid passport copy before start.
- I9 attestation and employment application required for clearance.
- Background checks including 7-year location/work history, EPLS/GSA/SAM, and HHS/OIG checks, updated annually.
- Passport-style photo for badge.
- Personal or auto insurance and auto registration documentation required and must be kept current during assignment.
- Physical exam within 12 months of start date.
- Immunizations and health screenings:
- TDAP vaccine within past 10 years (no declinations accepted).
- Negative TB test or chest X-ray within 1 year; if past positive TB, annual TBQ and chest X-ray within 5 years required.
- Proof of immunity or vaccination for Measles, Mumps, Rubella (MMR) for candidates born in 1957 or later.
- Hepatitis B titer or vaccine series; declination accepted if within 1 year of start.
- Varicella immunity or vaccination; no declinations accepted.
- Negative 9-panel drug screen within 1 year of start.
- Seasonal flu vaccination or declination accepted.
- COVID vaccination proof or medical/religious exemption required prior to start.
- Review and sign facility-specific policies including training content acknowledgment and hand hygiene policy prior to start.
- Two professional references required.
- Annual skills checklist required.
Additional Notes
- Hours worked are based on completed visits (25 visits = 40 hours).
- Locals are eligible to apply.
- Rate may vary based on coverage territory and experience.
