Registered Nurse (Home Health RN)

Location: Elizabethtown, Kentucky
Contract Length: 13 weeks

Job Summary

• Provide home health nursing care for adult patients across multiple counties (Marion, Washington, Taylor, LaRue, Hardin, Meade, and Breckenridge).
• Case manage roughly 30 adult patients daily, completing approximately 6.5 points per day.
• Responsible for start of care (SOC), recertifications, and daily visits.
• No clinical involvement beyond nursing duties; profiles go directly to the manager for consideration.

Schedule

• Day shift: Monday to Friday, 8:00 AM to 5:00 PM (40 hours/week).
• Weekend rotation required, hours 8:00 AM to 5:00 PM.
• Holiday rotation participation required.
• No on-call responsibilities.

Orientation

• Two weeks of orientation with a preceptor, including a few visits with a work coach.

Required Qualifications

• Valid RN license in Kentucky.
• Home Health experience (1 year of experience).
• Home Health experience within the last year.
• BLS.
• Associate of Science in Nursing / Associate Degree in Nursing.
• Med Surg experience.
• Experience with Homecare Homebase.

Additional Information

• No experience required in managing home ventilators.
• Support available 24/7 from clinical manager or educator for emergencies; 8 AM to 5 PM for routine support.
• Dress code: Scrubs (no specific color requirement).
• Equipment provided: Phone and tablet.

Patient Population

• Adults only.

Coverage Area

• Marion, Washington, Taylor, LaRue, Hardin, Meade, and Breckenridge Counties.
