Description
Case Manager - Hospice
Location: Grand Junction, Colorado
Employment Type: Contract
Contract Length: 13 weeks
Shift: Monday to Friday, 8:00 AM – 5:00 PM
Hours per Week: 40
Job Description
- Provide compassionate hospice nursing care, supporting patients and families with strength and courage.
- Serve as a primary nurse with an assigned caseload, leading the interdisciplinary team for those patients.
- Deliver care in various settings including private homes, nursing homes, group homes, and assisted living facilities.
- Act as a teacher to patients and caregivers, helping identify personal goals of care and building caregiver confidence.
- Manage symptom control and perform comprehensive assessments requiring critical thinking.
- Enjoy flexibility in daily scheduling to support work/life balance.
- Occasional weekend and on-call backup required.
Required Qualifications
- Valid Registered Nurse (RN) license in Colorado
- Minimum of 2 years of Hospice Palliative Care RN experience, including experience within the last year
- Basic Life Support (BLS) certification
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
