Physical Therapist - Home Health (Travel/Contract)

Location: Sarasota, Florida
Contract Length: 20 weeks
Shift: Day shift, 8:00 AM – 5:00 PM (5 days/week, 40 hours/week)

Job Description

• Provide skilled home health physical therapy visits (6-8 visits per day) for patients with conditions including med/surg, cardiac, ortho (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, Parkinson’s, trauma, amputations (AKA, BKA), respiratory patients, neuro, weakness, falls, and failure to thrive.
• Perform assessments and treatments involving cardiopulmonary/circulatory systems, orthopedic and neurological rehabilitation, use of equipment and devices (gait belt, rolling walker, sit-to-stand lift, sliding boards, collars, supplemental oxygen, prosthetics, wheelchair assessment).
• Develop and implement home exercise programs.
• Provide patient and family education, including safety and injury prevention.
• Document thoroughly using electronic systems.
• Participate in billing processes.
• Weekend work required 1-2 weekends per contract.

Required Qualifications

• Valid Physical Therapist license in Florida.
• Minimum 1 year of Physical Therapist experience, including experience within the last year.
• Experience in a Home Health setting.
• Basic Life Support (BLS) certification.
